(Page 2) <br />MINE ID # OR PROSPECTING ID #: M-19Rn-9aa <br />INSPECTION DATE: 11/1£i/1 p INSPECTORS INITIALS: -RK <br />OBSERVATIONS <br />The Division conducted a monitoring and construction progress inspection off the site on <br />11/17/10. Present during the monitoring inspection of the site was Mr. Jeff Campbell. <br />Inspected the under drain discharge area. Both the south under drain and 4 inch pipes <br />were discharging at about the same rate as was noted during the last inspection. All the <br />lower ponds and surface water diversion sumps were dry. Inspected AG1.5 (the NPDES <br />Discharge flume) it was dry. <br />Continued the inspection to the Phase I,II,III&IV Valley leach facility and collected the <br />following readings from the High and Low Volume Solution Collection transducers. <br />Phase I high volume solution collection transducer # 1 was recorded at 32.2 #2 was at <br />46.9 and # 3 pressure transducer reading was not functioning. The bore hole (piezometer) <br />for the pond was at 52.0 and the new caisson was at 41.0.Low volume solution collection <br />transducer # 1 was recorded at 0.41 and # 2 was at 0.45. <br />Phase II&III High Volume solution collection pressure transducer # 4 was recorded at 41.9 <br />#5 was at 37.5 and # 6 was at 45.1 Piozemeter for the pond was at 46.4. Low volume <br />solution collection pressure transducer # 1 was recorded at 41.9 #2 was at 37.5 and # 3 <br />was at 45.1.Piezomter for the pond was at 46.4. Low volume solution collection transducer <br /># 1 was recorded at 0.56 and # 2 was at 0.14. Piezemeter for the pond was recorded at <br />46.4. <br />Phase IV high volume solution collection pressure transducer # 7 was recorded at 48.6 # 8 <br />was recorded at 47.6 # 9 was at 47.9 and # 10(pipe) was recorded at 50.2. Low volume <br />solution collection transducer # 1 was recorded at 15.4 and # 2 was at 12.2 inches of <br />hydrostatic head. <br />Inspected the external storage pond, it was holding a partially frozen water. Level was <br />about the same as was noted during the past inspection. Low volume solution collection <br />transducer # 1 was recorded at 13.3 and # 2 was recorded at 13.8 inches of hydrostatic <br />head. <br />The Division observed some solution ponding along the third lift of the valley leach <br />facility, which is not allowed under the approved permit. CC&V were notified immediately <br />to correct the problem and the individual in charge of the valley leach facility came up <br />to the area and started the remedial action to percolate the solution. The Division <br />understands the need to relive pressure during winter months, and have in the past <br />allowed the buildup of ice to decrease the water passing through the drip irrigation <br />system. However, significant solution collection at the surface of the pad for extended <br />periods is not allowed. Because of the significant amount of ponding on the pad, it will <br />be cited as a possible Problem. <br />Inspected the Phase IVC perimeter ditch, it was found to be in good repair. <br />Inspected the Cresson mine, it was active. Haul road drainage control structures were <br />found to be in good repair. <br />After the valley leach facility inspection, met up with Timm Comer, Joe Hickey, Jim Smith <br />and Tom Shepard, and proceeded to inspect the Phase V valley leach facility construction <br />area. A small area of the slopes along the North side that were already lined but not yet <br />certified was being covered with drain cover material. The High volume and low volume <br />solution collection pressure transducers electrical hook up was being put together at the <br />time of the inspection. The Phase V pad area construction should be completed and <br />buttoned up by the end of the week.
