(Page 2) <br />MINE ID # OR PROSPECTING ID #: M-1977-417 <br />INSPECTION DATE: 1 1.417410 <br />OBSERVATIONS <br />INSPECTORS INITIALS: R_ <br />The Division conducted a pre operational inspection of the site on 11/17/09. The <br />applicant submitted a conversion application from the current 110 mine covering 9.9 acres <br />to a 112 permit application covering 93.25 acres. <br />Site is an old Clay and sand and gravel operation located approximately 5 miles east <br />north east of Colorado City Colorado. Present during the inspection was Mr. Joe Gagliano <br />of L.J Development. <br />This inspection concentrated on the acres being added to the permit. The current 110 <br />permit was not inspected because it was already inspected on 10/26/10.It was active at <br />the time of the inspection. <br />The entire amended area was driven and inspected. The operator had placed appropriate <br />permit boundary markers properly delineating the acres that are being added to the <br />current permit. <br />Along the west side of the acres being added outside of the permit area there is a very <br />large stock pond. Even if the operators mines in this area below the invert of the <br />channel the potential for impacting the drainage that feeds this pond is considered to be <br />minimal, given the set back from the pit area to the drainage channel. <br />There is another dry channel along the east side of the permit area that is increased <br />that also feeds another stock pond. The channel has the potential to be impacted. <br />Operator agreed to 100 foot setback from the center of the channel to the pit boundary. <br />The set back should be sufficient to address any potential impacts to this dry channel. <br />With the exception of a poorly maintained fence, there are no other manmade structures <br />with 200 feet of the additional acres. <br />Permit conversion application sign was properly posted at the access to the 110 permit. <br />The site was found as described in the conversion application. <br />cc: F CE <br />I & E Contact Address <br />F BL <br />NAME: - MR JOE GAGLIANO F FS, <br />OPERATOR: J DEVELOPMENT r HW <br />F HMWMD (CH) <br />STREET: 56 NORTH MERRIL DR F SE <br />CITY/STATE/ZIP: PU RI 0 WEST C'O 81007 r WQCD (CH)
