Inc. <br />Permit No. M-2006-039 North Clear Creek Construction Materials Mine <br />Garfield County, Colorado <br />Dear Mr. Dupler: <br />In accordance with the regulations of the Division of Reclamation, Mining and Safety, Chevron <br />U.S.A. Inc. hereby files its annual report and annual fee for the subject mine. <br />Preceding Year <br />The North Clear Creek Construction Materials Mine was opened in March of 2008 and the mine <br />has been in operation since that time. From November 2009 to end of October 2010, a total of <br />16,745 tons or 22,605.75 cubic yards of material has been removed. A noxious weed control <br />program has been in place since inception of the mine and stormwater management controls are <br />in place and monitored regularly. <br />There have been no changes to the previous year's area of disturbance or no additional <br />reclamation activities have taken place, therefore the map provided with last year's annual report <br />is still accurate and representative of current mine conditions. For that reason, no map is included <br />with this report. <br />A check in the amount of $323.00 for the annual fee is enclosed. <br />
