PARTI <br />Page No. 7 <br />Permit No. COG-070000 <br />(i) The date, type, exact place, and time of sampling or measurements; <br />(ii) The individual(s) who performed the sampling or measurements; <br />(iii) The date(s) the analyses were performed; <br />(iv) The individual(s) who performed the analyses; <br />(v) The analytical techniques or methods used; and <br />(vi) The results of such analyses. <br />b) Thepermittee shall retain for a minimum of three (3) years records of all monitoring information, including all <br />original strip chart recordings for continuous monitoring instrumentation, all calibration and maintenance <br />records, copies of all reports required by this permit and records of all data used to complete the application for <br />this permit. This period of retention shall be extended during the course of any unresolved litigation regarding <br />the discharge of pollutants by the permittee or when requested by the Division. <br />5. Additional Monitoring by Permittee <br />If the permittee, using the approved analytical methods, monitors any parameter more frequently than required by this <br />permit, then the results of such monitoring shall be included in the calculation and reporting of the values required in <br />the Discharge Monitoring Report Form or other forms as required by the Division. Such increased frequency shall also <br />be indicated. <br />6. Flow Measuring Devices <br />Flow measuring and metering shall be provided to give representati <br />ve values of throughput and treatment of the <br />wastewater system. Unless specifically waived in the certification, the metering device shall be equipped with a local <br />flow indication instrument and a flow indication-recording-totalization device suitable for providing permanent flow <br />records, which should be in the plant control building. <br />At the request of the Director of the State Water Quality Control Division, the permittee must be able to show proof of <br />the accuracy of any flow-measuring device used in obtaining data submitted in the monitoring report. The <br />flow-measuring device must indicate values within ten (10) percent of the actual flow discharging from the point <br />source. <br />Contamination: <br />If groundwater contamination is encountered, then the permittee is to contact the Division and, if appropriate, the <br />owner of the collection system receiving the discharge. Since the discharge of contaminated groundwater is not <br />covered under this permit, the permittee shall immediately apply for groundwater remediation certification <br />i.?+-.//......... ..A-1- +n+._ .. .. /..-_rn -:.-rr_:.ir___2? i,.n n ... <br />capabilities? <br />REPORTING <br />environmental response <br />Si atory Requirements <br />All reports, applications, or information required for submittal shall be signed and certified for accuracy by the <br />permittee in accord with the following criteria: <br />a) In the case of corporations, by a principal executive officer of at least the level of vice-president or his or her <br />duly authorized representative, if such representative is responsible for the overall operation of the operation <br />from which the discharge described in the form originates; <br />b) In the case of a partnership, by a general partner; <br />c) In the case of a sole proprietorship, by the proprietor; <br />d) In the case of a municipal, state, or other public operation, by either a principal executive officer, <br />ranking elected official, or other duly authorized employee. <br />2. Monthly Reports <br />Monitoring results shall be for each calendar month and reported on the DMR forms (EPA forms 3320-1). DMR <br />forms shall normally be submitted on a monthly basis; however the Division may determine a different frequency. The <br />forms shall be mailed to the Division at the address listed below so that they are received by the Division no later than <br />the 28th day of the following month. If no discharge occurs during the reporting period, "No Discharge" shall be <br />reported. <br />The DMR forms shall be filled out accurately and completely in accordance with the requirements of this permit and <br />the instructions on the forms, and shall be signed by an authorized person as identified in the preceding section, Part <br />I.F.1. <br />The DMR forms consist of four pages - the top 'original' copy, and three attached no-carbon-required copies. After <br />the DMR form has been filled out and signed, the four copies must be separated and distributed as follows. <br />Revised 7/21/2008
