ATTACHMENT TO ANNUAL REPORT 11- - w VAN 4 MINE f, PERMIT NO. M-1997-032 November 1, 2010 No mining occurred at the Van 4 in 2009 or 2010; however, Denison is currently evaluating the Van 4 Mine. Prior to initiating mining activities, the surface facilities at the Van 4 will need to be rehabilitated; however, the existing Division permitted boundary will not be changed. Prior to initiating mining activities, the Division will be formally notified of any site disturbance activities at the Van 4 Mine. In addition, a vent hole was reopened in May of 2007; however, it was determined that the hole was not useable, and it was resealed and reclaimed. Vegetation status of the vent hole was evaluated in September of 2010, and bond release was requested from the division; however, some confusion about the vent location occurred during the Division site visit and Denison will clarify with a map and second release request in the spring of 2011. In March of 2008, the Division provided notification to Denison Mines that the Van 4 Mine is a Designated Mining Operation. In May of 2009, Denison submitted an Environmental Protection Plan (EPP) for the Van 4 Mine. In September of 2009, Denison received comments on the EPP from the Division, and Denison addressed these comments in March of 2010. Conditional acceptance of the EPP was received from the Division in June of 2010 and a response was submitted accepting the conditions in August of 2010. No changes occurred in the Permit M-1977-032 area during 2010, and the site map, submitted with the Permit application, on file with the Division, remains current. Stormwater evaluations were completed in 2009, and an assessment of needed maintenance was also completed. Stormwater improvement work will be conducted in November of 2010. No reclamation work was conducted during 2010, and no reclamation is scheduled during 2011.
