PERMIT #: M-1998-014 <br />INSPECTOR'S INITIALS: DMC <br />INSPECTION DATE: October 26, 2010 <br />OBSERVATIONS <br />This inspection was conducted as part of the Colorado Division of Reclamation, Mining and Safety (DRMS) normal <br />monitoring program. Sean Frisch, representing the Operator, was present during the inspection. Photographs are included <br />at the end of this report in order to illustrate some of the conditions observed during this inspection. <br />The Gypsum Ranch Pit is located approximately 1.5 miles east of Gypsum, and accessed from U.S. Highway 6. The <br />permitted area includes 155.6 +/- acres. It is a phased operation, consisting of six phases, mining in an easterly direction. <br />Active mining was occurring in phase 4 during the time of this inspection. An active concrete plant is located in the west <br />portion of the permit area, in the northeast corner of the phase 1 area. The asphalt plant had been removed prior to this <br />inspection, with only minor remnants left to be disposed of. <br />An adequate mine identification sign was located at the mine site entrance. Fencing adequately delineates the affected area <br />boundary at the site. <br />Fuel storage tanks observed on site appeared to have adequate secondary containment structures. <br />No evidence of erosion or off-site damage was noted during this inspection. <br />No noxious weeds were noted during this inspection. <br />No problems or violations were noted during this inspection. <br />Responses to this inspection report should be directed to Dustin Czapla at the Division of Reclamation, Mining and <br />Safety, Grand Junction Field Office, 101 South Yd Street, Room 301, Grand Junction, Colorado 81501, phone number <br />(970) 243-6299. <br />Inspection Contact Address <br />Sean Frisch <br />Lafarge West, Inc. <br />P.O.
