(Page 2) <br />MINE ID # OR PROSPECTING ID #: m-9nn3-mi <br />INSPECTION DATE: 09/99/10 <br />OBSERVATIONS <br />INSPECTORS INITIALS: RK <br />The Division conducted an inspection of the site as part of sites to be inspected during <br />the fiscal years 2010-2011, Present during the inspection were Dr Angela Bellantoni, the <br />land owners consultant and Mr. Mike Ausburn, of Pioneer Sand, who is operating the site. <br />1) Site is located approximately 5 miles south East of the Holcim Portland Cement <br />Plant. <br />2) Site was active at the time of the inspection, with front end loader, loading haul <br />trucks and a dozer working the active mining area in Phase III. The total permitted <br />acres for the site is 920 rather than 930 as reported in the 2007 inspection <br />report. The operator amended the permit in 2004 by adding 680 acres to the existing <br />240 acres. <br />3) Inspected the Phase I area, all the mining in this area is completed and a partial <br />reclamation had taken place. A small erosion feature was noted in the east facing <br />slopes that Mr. Ausburn agreed to address while conducting reclamation in the area <br />before winter. Berm along the perimeter of the affected area was found to be in <br />good repair. Site is allowed to affect a total of 60 acres at any given time. <br />4) Phase III as stated was active at the time of the inspection. Berm along the <br />perimeter of the active mining area was in good repair. All drainage from the <br />active mining area along the North West is routed to the south eastern pit area <br />that is already mined out. High wall was stable and no surface failure noted. The <br />affected area along the North west side of the active pit area has been graded to <br />almost 5:1 slope. A couple of processed stockpiled materials were noted along the <br />north side of the active pit area. <br />5) In the 2007 inspection the Division had noted a possible problem with a total area <br />that as affected between Phases I and III. The operator was requested to provide a <br />surveyed map of the affected area to make sure it did not exceed the maximum <br />affected area for the site. On 10/30/2007 the requested survey of the affected area <br />was submitted. It showed Phase I had a total affected area of7.1 acres and the <br />current mining area of Phase III showed total of 6.3 acres.
