That is part of the permit process.
Yes, the applicant can be JBS Five Rivers Cattle Feeding Co.
Yes, the permit application does require the submittal of a traffic control plan. We have to review and approve it prior to issuing the permit. However, if you are not
planning on doing work on CDOT ROW, you can write a letter stating that a traffic control plan was not required since all the work will be taking place on private
property. If you can bore on both sides from private property, you don't need to have a traffic control plan.
JBS Five Rivers Cattle Feeding Co. can provide a copy of insurance including CDOT in its coverage. However, once you have a contractor on board, we need to have
him added to the permit and have a copy of his insurance.
Please submit the permit application to: mark.nusskerniRdot.state.co.us
Thanks.
From: JOSEPH GAGLIANO (mai o jpg0incas0 Dml
Sent: Thursday, September 23, 2010 10:55 AM
To: Trevizo, Joe
Cc: Self Shane
Subject: Re: Utility Permit 287-US 50
JOE TREVIZO,
DO WE NEED TO DO THE CONSTRUCTION STORMWATER PERMT, BEFORE WE START CONSTRUCTION?
WE ARE PLANNING ON THE APPLICANT BEING IBS FIVE RIVERS CATTLE FEEDING ON THE CDOT FORM 1233 IS THIS OKAY?
JBS IS THE LAND OWNER.
THE NEXT QUESTION IS DO WE HAVE TO HAVE A TRAFFIC CONTROL PLAN BEFORE THE CONSTRUCTION CONTRACT IS EXECUTED?
THE WORK CONSIST OF A BORING UNDER THE HIGHWAY. CDOT Utilities can't legally sign agreements, and is only authorized
to
issue utility permits. In order to avoid possible delays in involving the AG's Office, we can issue the Five Rivers Cattle Co. a utility permit to trench under US-50. I will also attach
a cover
letter to the permit stating'THE MINING OPERATION WILL NOT ADVERSELY AFFECT THE STABILITY OF ANY SIGNIFICANT, VALUABLE AND EPRMANENT MAN-MADE
STRUCTURE LOCATED WITHIN TWOO HUNDERED (200) FEE OF THE AFFECTED ALND". The issued permit and cover letter should cover all the requirements listed on the letter
from the Division of Reclamation, Mining and Safety.
In order to get this permit processed, we will need insurance naming CDOT under it's coverage, traffic control plan, brief explanation of the work, project plans (we may have them on
file), and a copy of CDOT Form 1233 (Utility Permit). I have attached a copy of the permit application for your convenience. Please see page two of the permit application for further
information.
Mark Nusskern has been working on this permit since April 2009.
