The storm water permit has to be submitted with the utility permit application. That is part of the permit process.
Yes, the applicant can be JBS Five Rivers Cattle Feeding Co.
Yes, the permit application does require the submittal of a traffic control plan. We have to review and approve it prior to issuing the permit. However, if you are not
planning on doing work on CDOT ROW, you can write a letter stating that a traffic control plan was not required since all the work will be taking place on private
property. If you can bore on both sides from private property, you don't need to have a traffic control plan.
JBS Five Rivers Cattle Feeding Co. can provide a copy of insurance including CDOT in its coverage. However, once you have a contractor on board, we need to have
him added to the permit and have a copy of his insurance.

THE NEXT QUESTION IS DO WE HAVE TO HAVE A TRAFFIC CONTROL PLAN BEFORE THE CONSTRUCTION CONTRACT IS EXECUTED?
THE WORK CONSIST OF A BORING UNDER THE HIGHWAY. THE PLANS WILL BE SUPPLIED IF YOUR OFFICE DOES NOT HAVE THEM.
CAN JBS FIVE RIVERS FEEDING PROVIDE THE INSURANCE COVERAGE?

In order to get this permit processed, we will need insurance naming CDOT under it's coverage, traffic control plan, brief explanation of the work, project plans (we may have them on
file), and a copy of CDOT Form 1233 (Utility Permit). I have attached a copy of the permit application for your convenience. Please see page two of the permit application for further
information.
Mark Nusskern has been working on this permit since April 2009. The person he has been dealing with is Shane Self, with Enviro-Ag.
