October 19,2010 C-1982-057/Seneca II-W Mine SLB <br /> embankment. The operator needs to monitor this situation and ensure that the integrity of the dam is not <br /> compromised. The rip rap in the emergency spillway of Pond 005 has weathered and will need to be replaced to <br /> meet design standards. <br /> Pond 009 held water but was not discharging. The embankment was stable and vegetated,but there was quite a bit <br /> of thistle on and around the embankment. <br /> Pond 015 was discharging clear water at approximately 2-3 gpm. The embankment, inlet,and outlet were stable <br /> and vegetated. <br /> Pond 017 was discharging clear water. The embankment was stable and vegetated,but there is also a considerable <br /> amount of thistle on this embankment. <br /> ROADS—Rule 4.03 L]f]Construction 4.03.1(3)/4.03.2(3)❑❑Drainage 4.03.1(4)/4.03.2(4)110 Surfacing and <br /> Maintenance4.03.1(5)and(6)/4.03.2(5)and(6)0❑Reclamation 4.03.1(7)/4.03.2(7): <br /> The operator was regrading Haul Road A to downsize it to the approved postmining width. Grading was <br /> observed between the shop area and the north side of Pond 006. <br /> An excavator was working in the Road J culvert outlets(J-1, -3, -4, -5, -6,and-7)during the inspection. The <br /> operator installed new silt fence at each outlet,excavated a small sump in front of each fence,and installed rip rap <br /> at some of the culvert outfalls. The silt fence installation was good;the rip rap consisted of large(24"-30") <br /> boulders with no gradation to smaller material. If these channels are not stable through the spring runoff,the <br /> operator should consider reinstalling the rip rap with a gradation of material. <br /> Culvert Y-2,at the shop entrance,needs to be cleaned out; some sediment has accumulated at the outlet end. <br /> Culvert G-5 needed to be cleaned out and the ditch needed maintenance. The operator indicated, after the close of <br /> the inspection,that maintenance at this culvert was underway on October 21,2010. <br /> The Tie Across Haul Road was regraded during the inspection. The road surface and ditches were in good <br /> condition. <br /> SLIDES and DAMAGE -Rule 4.12: <br /> The Pond 016 slide area was saturated,with ponded water throughout. The operator had cut a channel to <br /> drain the water from the Haul Road J roadway and from within the slide itself. It appeared that there was a <br /> considerable amount of groundwater flowing through the slide area. <br /> The operator indicated that they will likely proposed removal of all of the slide material adjacent to Pond 016, <br /> installation of underdrains at the toe of the slide,regrading/reclamation of Pond 016,and relocation of Road J to <br /> remediate the most recent slide. The operator will be resubmitting the revision(initial submittal was incomplete) <br /> for the slide remediation plan. <br /> Number of Partial Inspection this Fiscal Year: 1 <br /> Number of Complete Inspections this Fiscal Year: 2 <br /> Page 3 of 9 <br />
