Application Incomplete <br />Dear Mr. and Mrs. Sikes: <br />On October 14, 2010, the Division of Reclamation, Mining and Safety received your 112 Construction Materials <br />Reclamation Permit Application package for the Carrizo Stone North, File No. M-2005-067. Preliminary review of <br />the information received determined that the following items must be corrected before the Division can consider your <br />application as being submitted and technical review can begin: <br />APPLICATION FORM: <br />1.) Item #1 of the first page of the application is incorrectly filled out. The applicant/operator or company name <br />should be "Carrizo Stone, LLC" spelled exactly the same way as it is filled with the Colorado Secretary of <br />State. Enclosed with this letter is a copy of the first page of the application with this mistake highlighted. <br />Please submit a replacement copy of this page of the application with the correct information filled out. <br />2.) Please clarify Item #2 of the first page of the application. The current operation name is "Carrizo Stone <br />North", however the conversion application indicates the operation name is "Raymond & Stephanie Sikes". <br />The Division assumes you wish to keep the current site name of "Carrizo Stone North." Please submit a <br />replacement copy of this page of the application with the correct information. <br />3.) Item #3 of the first page of the application indicates the current permit area is 9.9 acres. Item 3.1 indicates the <br />operator wishes to increase the permit acreage by 30 acres, however item 3.2 indicates the total permit area is <br />30 acres. Based on the dimensions of the permit boundary depicted in the Exhibit C, mining plan map, the <br />permit acreage is depicted at 28 acres. Please clarify how many acres you are requesting to be included in the <br />permit application. This acreage amount must be consistent throughout the entire permit application. For <br />item 3, please determine what the actual permit acreage will be. If the actual permit area is 28 acres. Item 3.1 <br />should be 18.1 acres yielding a total permit area of 28 acres for item 3.2. Please submit a replacement copy of <br />this page of the application with the correct information. <br />4.) Page 8 of the application page needs to be corrected. The applicant/operator or company name should be <br />"Carrizo Stone, LLC" spelled exactly the same way as it is filled with the Colorado Secretary of State. This <br />error must also be corrected on the notary block at the bottom of the page. Enclosed with this letter is a copy <br />of the eighth page of the application with these mistakes highlighted.
