No operator representative accompanied during the inspection, <br />however I spoke with HR/Administrative Assistant April Shults and Safety Manager John Barton at the mine <br />office prior to the inspection. Weather conditions were clear and warm, and ground conditions were dry; no recent <br />precipitation. Focus of the inspection was on interim stabilization vegetation in the vicinity of the temporary <br />waste site, topsoil and construction material stockpiles. <br />HYDROLOGIC BALANCE - Rule 4.05 <br />Drainage Control 4.05.1, 4.05.2, 4.05.3; Siltation Structures 4.05.5, 4.05.6; Discharge Structures 4.05.7, <br />4.05.10; Diversions 4.05.4; Effluent Limits 4.05.2; Ground Water Monitoring 4.05.13; Surface Water Monitoring <br />4.05.13; Drainage — Acid and Toxic Materials 4.05.8; Impoundments 4.05.6, 4.05.9; Stream Buffer Zones 4.05.18: <br />Drainage and sediment control structures in the temporary waste disposal area, soil /construction material <br />stockpiles area and office facility area were observed and appeared to be properly maintained and functional. The <br />sediment pond was not discharging; water level in the pond was approximately 3 feet below the discharge level. <br />The office area containment berm and SAE silt fence were in good condition, as was the SAE silt fence at the <br />outlet of the construction material stockpile containment ditch. The operator had a track hoe on site was cleaning <br />sediment from culvert inlets, the in -ditch sump, and silt fence ditch outlets along the lower road at the time of the <br />inspection. <br />Number of Partial Inspection this Fiscal Year: 3 <br />Number of Complete Inspections this Fiscal Year: 1 <br />Page 2 of 7 <br />
