(Page 2) <br />MINE ID # OR PROSPECTING ID #: m-9nn9-n9A <br />INSPECTION DATE: 9199/in INSPECTORS INITIALS: _gK <br />OBSERVATIONS <br />The Division conducted an inspection of the site as part of sites to be inspected in <br />fiscal years 2010-2011. <br />1) Mine is located approximately 25 miles North West of Pueblo Colorado. <br />2) Total permitted acres is 640, with maximum areas to be affected through the life <br />of mine limited to 120 acres. <br />3) Maximum acres to be affected through each phase not to exceed 15 acres. <br />Commodities to be mined consist of Sand stone slabs, limestone and sand and <br />gravel. Site consists of pre- law disturbed areas. <br />4) The last time the site was inspected in 2003, via an aerial photo, the warranty <br />was not posted and permit was not issued. <br />5) Present during the site visit on 9/29/10 was Dr. Angela Bellantoni, the <br />operator's consultant. <br />6) Permit sign properly posted at the access to the property. A small portable <br />scale and mobile trailer noted along the South side of the access road. <br />7) A small amount of sand stone and limestone slabs noted scattered throughout the <br />affected area. Site was not active at the time of the inspection, but according <br />to Angela, it was active last year, which was evident by the scattered lime <br />stone and Sandstone slabs. <br />8) The current warranty in the amount of $43,500.00 is more than adequate to cover <br />the current affected area of approximately 11 acres. However, it would be <br />advantageous for the operator to clearly delineate the total amount of acres <br />that were affected pre- mining to limit future liability. In addition given the <br />difficulty of reclaiming such sites, operator will need to notify the Division <br />if full scale mining activity commences so the warranty can be adjusted. <br />9) There is a perennial drainage that is located along the south west side of the <br />active and pre- law affected and active mining areas. It did not appear it had <br />been affected by the mining activity since the active mining area is further <br />away from the stream.
