(Page 2) <br />MINE ID # OR PROSPECTING ID #: J\A-1991-118 <br />INSPECTION DATE: 9/99.11n <br />OBSERVATIONS <br />INSPECTOREIS INITIALS: _RK <br />The Division conducted a construction progress inspection of the Phase V valley leach <br />facility on 10/16/10.Presnt during the inspection were Timm Comer and Tom Shepherd of <br />CC&V. <br />The Phase V pregnant pond area should be completed with two feet of drain cover in place <br />by the end of next week. Certification with as built diagrams to DRMS, soon after. To <br />date 3.6 million square feet of geomembrane liner is deployed in the pregnant solution <br />collection area. The high pressure and low pressure pipes are in place. The frame for the <br />shed that will house the electrical connection apparatus for the pumps was in place along <br />the North east side of the riser pipes. <br />Drain cover was being pushed along the south facing slopes of the pregnant solution <br />collection area. As stated drain cover placement should be completes by the end of the <br />week. Amec, QA/QC was on site supervising the placement. <br />Geomembrane liner and clay liner were being deployed along the south facing slopes of the <br />ore storage area. CC&V plan to commence liner deployment, until the end of November, <br />weather permitting. <br />A small amount of processed clay stockpile was noted along the western side of the <br />pregnant solution collection pond area. In anticipation of drain cover deployment, a 20 <br />foot road with drain cover material was being constructed in the same area. A minimum of <br />20 foot cover is needed to allow the trucks to go over the deployed liner. <br />Crusher was active at the time of the inspection. CC&V also continues to apply lime <br />slaking in selected area of the exiting pad to increase the ph and facilitate better <br />recovery.
