The issued permit and cover letter <br />should cover all the requirements listed on the letter from the Division of Reclamation, Mining and <br />Safety. <br />In order to get this permit processed, we will need insurance naming CDOT under it's coverage, <br />traffic control plan, brief explanation of the work, project plans (we may have them on file), and a <br />copy of CDOT Form 1233 (Utility Permit). I have attached a copy of the permit application for your <br />convenience. Please see page two of the permit application for further information. <br />Mark Nusskern has been working on this permit since April 2009.
