Page 2 of 3 <br />6.3.2 EXHIBIT B - Site Description <br />Essentially adequate, although I believe it may be worth pointing out just for clarification that there is an existing pre- <br />permit high-wall along the northeast side of the site that is outside the permit boundary. I think it would also be worth <br />adding this feature to the mining and reclamation plan maps just to avoid possible confusion in the future. <br />6.3.3 EXHIBIT C - Mining Plan <br />Adequate as submitted <br />6.3.4 EXHIBIT D - Reclamation Plan <br />Weed control methods are required to be used for all noxious weeds on site, and a generic plan to control all noxious <br />weeds should be included in all permit applications. If a specific noxious weed species appears in the future, the plan may <br />need to be modified to deal with that particular species at that time. <br />Again, I believe it may be worth pointing out for clarification that there is an existing pre-permit high-wall along the <br />northeast side of the site that is outside the current permit boundary and specify if this area will be impacted by proposed <br />reclamation activities associated with this permit. <br />The reclamation costs are still under review at this time. Your estimate appears to be well prepared; however, the division <br />is required to complete a separate bond estimate as well. Once all adequacy issues are addressed, this will be completed. <br />6.3.5 EXHIBIT E - Map <br />The maps with the proposed permit boundary should be modified to show that the affected area/permit boundary will not <br />be within 200 feet of the ditches to the south of the site, unless a damage waiver is obtained from the ditch owners. It <br />would appear that this would require the removal or modification of the "tail" on the south end of the proposed permitted <br />area. You may also want to take another look at the contouring on the reclamation plan map - as submitted it would <br />appear to be a "pit", but on-site it seems to be more of a "ridge removal" with the ridge/high-wall on the north end of the <br />site, but most of the remaining area being at the same level as the access road. Otherwise - adequate as submitted. <br />6.3.6 EXHIBIT F - List of Other Permits and Licenses Required <br />Enclosed for your review are any comments that we have received from other agencies that DMG solicits for comment on <br />new applications. None have been received to date. <br />6.3.7 EXHIBIT G - Source of Legal Right-to-Enter <br />The copy of the property deed to Kristine Couch referred to in the package was not found. The Division will also need to <br />receive the notarized access agreement between property owner and permittee before the amendment can be approved. <br />6.3.8 EXHIBIT H - Municipalities Within a Two-mile Radius <br />Adequate as submitted <br />6.3.9 EXHIBIT I - Proof of Filing with County Clerk <br />Adequate as submitted <br />6.3.10 EXHIBIT J -Proof of Mailing of Notices to Board of County Commissioners <br />and Soil Conservation District <br />Adequate as submitted
