(Page 2) <br />MINE ID # OR PROSPECTING ID #: M-1980-014 <br />INSPECTION DATE: 21 Sept 2010 INSPECTOR'S INITIALS: _DR? <br />OBSERVATIONS <br />This was a routine inspection conducted by David Bird of DRMS. The Operator was contacted but did not attend <br />the inspection. The site is located about 13 miles east-southeast of Hartsel, and about'/2 mile west of the inlet to <br />Elevenmile Reservoir alongside County Road 59. The commodity of interest is sand and gravel. The total permit <br />area is 59.6 acres, and the post-mining land use is described as rangeland. No bond is held as the operator is a <br />government entity. <br />No problems were observed. The site was inactive at the time of the inspection and no equipment was on site <br />other than a couple of small screens. The only evidence of recent activity was three to four stockpiles of material <br />that appeared to have been placed on site within the past year, based on the extent of vegetation growth on the <br />piles. <br />No topsoil stockpiles were seen on site. The reclamation plan filed with the 1996 Conversion application <br />specifies that topsoil needed for reclamation will be hauled in and stockpiled as it becomes available from new <br />road projects where topsoil is removed. <br />The depth of excavation is shallow, so there is no exposure of ground water, no observable impacts to the <br />hydrologic balance, and no erosional issues were evident. <br />& E Contact Address cc: t CE <br /> F BLM <br />NAME: David Kintz F USFS <br /> IF HW <br />OPERATOR: Park County F HMWMD <br /> r OSE <br />STREET: P.O.
