The availability of records check list <br />is attached. <br />: Hydrologic Balance <br />Ponds inspected included the 004, 003, POCOCO Pond, and Pond 002. Pond 004 was discharging through a 8" <br />corrugated plastic pipe outside and smooth inside. Pond 003 was discharging through a small modified primary <br />spillway with a v-notched outlet. The POCOCO Pond was discharging through an 24" CMP. All ponds were <br />stable with no problems noted with the exception of the following: <br />- Pond 004 requires a trash rack at the inlet of the primary spillway. The trash rack was at the primary pipe but <br />not installed. <br />- Pond 004 embankments displayed willow trees growing in the embankments of the pond. These trees should be <br />removed to ensure the structural integrity of the embankments. <br />- Large animal burrows were indentified in the embankments of Ponds 003 and POCOCO. <br />- Sage brush shrubs were located in the emergency spillway of Pond 003. Theses shrubs will need to be removed <br />to ensure flow through the spillway. <br />- The inlet of the POCOCO Pond primary spillway has been damaged with a large amount of vegetation growth <br />surrounding the inlet. This will need repair and clear of vegetation. <br />- Thistle and Houndstounge rosettes were identified on the embankments of Ponds 003 and POCOCO. <br />Culverts A4, A5, A3, 33A, Al, 29A, 30A, 47A, 32A, 12A, 54A, 53A, 64A, 52A, 55A, 13A, 59A, 9A, 31A, A8, <br />and 15A were observed during this inspection. All culverts were stable with the exception of the following: <br />- an unidentified culvert was located perpendicular and - 10 feet from culvert 29A which is not located on Map <br />2.05.3 M4 <br />Number of Partial Inspection this Fiscal Year: 1 <br />Number of Complete Inspections this Fiscal Year: 1 <br />Page 2 of 7
