(Page 2) <br />MINE ID # OR PROSPECTING ID # M-1977-443 <br />INSPECTION DATE 09/1812006 INSPECTOR'S INITIALS DEG <br />OBSERVATIONS <br />This routine monitoring inspection of Permit Number M-1977-443 (Moon Pit No.2) was conducted by Deb E. G6nima of the <br />DRMS. Hardrock Sand and Gravel owner and representative Pat Weberwas present during this inspection. The pit is located <br />approximately 17 miles west of Craig, CO off of Highway 40 and currently consists of approximately 5 permitted acres where <br />approximately 5 acres are currently disturbed. <br />The secondary containment area for fuel storage in the active pit area is inadequate (see photo 3). It appeared that fuel was <br />being stored at the time of this inspection (see Problem No.1 on pg. 3). All storage tanks, petroleum and hazardous materials <br />on site for any period of time shall have secondary containment. The Operator must supply photo documentation that the fuel <br />tank and 5-gallon containers are stored in a proper secondary containment structure or are removed from the site on or before <br />December 1, 2006. A berm must be constructed around the tank. The dimensions of this berm must contain all contents of the <br />tank (when full) plus 10% of the tank's capacity. In addition, a liner shall cover the entire basin as a barrier to infiltration. <br />The mine entrance identification sign was legible, but the mine name and permit number were faded and should be made more <br />clearly visible per Rule 3.1.12(1)(c) (see photo 1). It also appeared that current disturbance was localized within the approved <br />permit area. However, since this site lacked permit boundary markers, this inspector was unable to verify that the operator is <br />mining within the approved permit boundaries. Therefore, per Rule 3.1.12(2) the operator must clearly mark the boundaries of <br />the affected area with monuments or other markers that are clearly visible and adequate to delineate such boundaries (see <br />Problem No. 2 and the corresponding corrective action noted on page 3 of this report). <br />The approved map does not clearly depict the current state of activity for the site. Therefore, a map clearly illustrating the <br />current site conditions is required (see Problem No. 3 and the corresponding corrective action listed on page 3). <br />Otherwise all activities and completed work at the time of inspection appeared to follow the guidelines and regulations set forth <br />by the state and the mining and reclamation plans for the pit, and no additional problems are noted for this site. The site is <br />stable, and there appears to be no offsite erosion, and no long-term (greater than 72 hours) water retention was observed. <br />During this inspection the operator stated that he intends to submit a request to increase the acreage for this site in the near <br />future. This further highlights the need for a detailed map of the current conditions at the site. <br />As a final note for future reference, this site is correctly positioned in the DMG online GIS. <br />Responses to this inspection report should be directed to Deb E.
