PERMIT #: M-1977-443 <br />INSPECTOR'S INITIALS: DMC <br />INSPECTION DATE: September 14, 2010 <br />OBSERVATIONS <br />This inspection was conducted as part of the Colorado Division of Reclamation, Mining and Safety (DRMS) normal <br />monitoring program. There was no company representative present during this inspection. Photographs are included at the <br />end of this report to illustrate some of the conditions observed during this inspection. <br />The following problems noted during a September, 2006 DRMS inspection remain unresolved at this time. To <br />avoid potential future enforcement actions for non-compliance with State Statutes Rules and Regulations, and/or <br />conditions specified in your permit for this site, you must submit compliance documentation for each of the <br />indicated problems within thirty days of the date of this report. <br />A map clearly identifying the current site conditions has not been submitted. A new map has been promised by <br />the operator in excess of four years now, and DRMS has yet to receive such a map. <br />2. The fuel tank stored on site lacks any form of secondary containment. All storage tanks, petroleum and any <br />hazardous materials on site for any period of time shall have appropriate secondary containment. The secondary <br />containment structures shall consist of an impermeable containment able to contain all contents of the tanks and <br />various containers (when full) plus 10% of the total capacity <br />3. The mine identification sign and affected area boundary markers are not posted pursuant to Rule 3.1.12(2). <br />The pit appeared to be inactive during this inspection. Several small stockpiles were observed in the pit floor, along with a <br />crusher and conveyor. The pit area was unreclaimed, with pit slopes left at approximately 1 H:1 V during this inspection. <br />The depth of the pit was approximately fifteen feet. <br />Due to the lack of affected area boundary markers at this site, it is unclear 'if all of the disturbance at this site falls within <br />the approved permit area. The site is approved for a maximum five acre disturbance. It appeared, during this inspection, <br />that nearly all of the approved area has been affected by mining operations. <br />The financial warranty currently held in the amount of $12,500 is considered adequate for this site at this time. <br />No other problems or violations were noted during this inspection. <br />Inspection Contact Address <br />Pat Weber <br />Hardrock Sand and Gravel <br />P.O.
