PERMIT #: M-1999-058 <br />INSPECTOR'S INITIALS: GRM <br />INSPECTION DATE: September 16, 2010 <br />collector ditch that runs down the access road to #5 was difficult to construct due to the narrowness of the road and <br />exposed bed rock in the road. The inspector and consultant noted some areas that needed to be bolstered to prevent a <br />blow out or short circuit but over all the grade and construction were good. The inspector advises the operator to mark the <br />collector ditch berms so that winter maintenance does not compromise the system for spring melt. <br />Overall the project is very well constructed and designed. The inspector must note that maintenance of the system is <br />paramount. Inspections should be done quarterly and after any major precipitation event. Failure to maintain the system <br />will result in unneccessary impacts on and off site. Off site damage will result in enforcement actions before the Mined <br />Land Reclamation Board. The operator should keep a log of stormwater system inspections noting the date and time, as <br />well as conditions of the sumps and ditches. Any repairs and maintenance should be recorded as well. This information <br />should be included in the permit's annual report to the Division. <br />The operator is now in complaince with the Board Order. The inspector and consultant noted six minor issues that need to <br />be addressed to finish up the process. With submission of the engineering report and documentation of addressing the <br />minor issues DRMS will be sending a "In Compliance" letter out. <br />The six issues noted are: <br />1) Clean out flagged portions of Ditch leading to Sump 5. <br />2) Add facilities sump to plan (no work on site) <br />3) Add vertical markers on ditch to Sump 5 for snowplowing <br />4) Dig entrance water bar and/or culvert into Sump 2. <br />5) Get 40 Ibs of seed (same as in the reclamation plan) for broadcast on the roads from Sumps 2, 3 and 4 as well <br />as berms and any other disturbance in the area.
