(Page 2) <br />MINE ID # OR PROSPECTING ID #: m-19Rn-244 <br />INSPECTION DATE: 2116110 INSPECTORS INITIALS: -RK <br />OBSERVATIONS <br />The Division conducted a monitoring construction progress and GW and under drain water <br />sampling inspection of the site on 9/16/10.Presnt during the inspection was Mr. Tim <br />Comer, manager Environmental Resources and Tom Shepherd manger of the Phase V leach <br />construction. <br />The inspection began in Arequa Gulch, where GW samples were collected for Sulfate and wad <br />cyanide analysis from compliance wells CRMW 3A AND 3B. Samples were collected in <br />accordance with EPA guidelines for water sampling. Four case volumes were purged before a <br />sample was collected. Operator collected a split using their own bottles. <br />while in the area, inspected the under drain. All lower ponds were dry. Water samples for <br />wad cyanide analysis, with a split for the operator were collected from the South under <br />drain, which was flowing at 8 gpm and 4 inch pipe or AG 1 which was flowing at 0.8 gpm. <br />inspection began around the Phase IVC perimeter liner inter phase ditch area. The west <br />facing pads were being leached at the time of the inspection. The inter phase ditch area <br />was walked and inspected and found to be in good repair. <br />Continued the inspection to Phase I pregnant pond and leach pad area area. It was active <br />at the time of the inspection. While in the area collected the following readings from <br />the high and low volume solution collection transducer for Phase I,II,III&IV. <br />Phase IV High Volume Solution Collection transducer # 7 was recorded at29.5 # 8 was at <br />29.4 #9 was at 28.9 and # 10 was at 29.8. Low volume solution collection transducer # 1 <br />was recorded at 12.4 and # 2 was at 10.4 inches of hydrostatic head. <br />Phase I high Volume solution Collection transducer # 1 was recorded at 22.4 # 2 was at <br />30.3 and # 3 was at 27.3 piezometer for the pond was recorded at 34.3.0. Low volume <br />solution collection transducer # 1 was recorded at .34 and # 2 was at .44. <br />Phase II & III High Volume Solution Collection Transducer # 4 was recorded at 38.2 #5 was <br />at 29.4 and # 6 was at 39.3. Low volume solution collection transducer # 1 was recorded <br />at.55 and # 2 was at.23 Piezometer for the pond was recorded at 41.7. Low volume solution <br />collection pressure transducer # 1 was recorded at 0.56 and # 2 was at 0.24 <br />Inspected the external pond it was holding about the same amount of water as noted during <br />the last inspection. Low volume solution collection transducer # 1 was recorded at <br />11.6and # 2 was at 9.6 inches of hydro static head. ' <br />Inspected the Phase V leach pad construction area. Leak Collection gravel along the <br />Eastern portion of the pregnant pond, drain cover along the west, and north side of the <br />pregnant pond and gemembarne along the south and North Eastern portion of the pregnant <br />pond area were being deployed at the time of the inspection. Amec QA/QC personnel were on <br />site observing the deployment as required. The western portion of the pregnant pond area <br />is almost covered with drain cover material.
