PERMIT #: M-1984-025 <br />INSPECTOR'S INITIALS: RCO <br />INSPECTION DATE: September 7, 2010 <br />OBSERVATIONS <br />This was a normal monitoring inspection, to verify whether there was any recent disturbance or reclamation and to see if <br />the noxious weeds have been treated. There was no evidence of any recent mining disturbance, and no reclamation, but <br />the Black henbane infestation finally appears to be getting under better control. There was no activity at the time of the <br />inspection, and there was no operator present. The gate was closed (though the support post needs to be repaired). The <br />required permit ID sign was posted near the gate and all boundaries were marked. <br />(BG) Backfilling and Grading: <br />Narrative: The working faces of the pits were still nearly vertical, and there was no backfilling or grading evident. <br />(FN) Financial Warranty: <br />Narrative: The bond amount is $55,081. This appears to be sufficient to reclaim the site under the current plan. <br />(HB) Hydrologic Balance: <br />Narrative: There is no stormwater runoff that leaves the site, and no exposed groundwater in the pit. However, there are <br />some small areas that have been excavated slightly deeper and exhibit near-wetland vegetation. The operator must not <br />excavate too deep <br />(MP) Gen. Compliance With Mine Plan: <br />Narrative: There was no equipment at the site. The operator has removed all fuel, debris, and contaminants, and keeps <br />the site fairly neat. Mining has resulted in pits on the SW and central parts of the site, that are less than 10 feet deep. <br />There were stockpiles of fines, topsoil, and pitrun. Less than half the site has been disturbed. <br />(OD) Off-site Damage: <br />Narrative: No activities have occurred outside the boundaries. <br />(RS) Reclamation Success: <br />Narrative: No reclamation activities have been carried out. <br />(RV) Revegetation: <br />Narrative: Undisturbed areas are mostly in sub-irrigated or dry rangeland vegetation. None of the disturbed areas have <br />been seeded, and most of those areas exhibit annual weeds. The areas that have been noted in past inspections as <br />containing infestations of Black henbane are mostly free of it now. However, there are still a few small areas with <br />scattered Black henbane plants remaining. These small infested areas were near the northern stockpiles (SE of the <br />entrance gate) and near the loadout (S of large CDOT stockpile). Please monitor and address these areas timely and <br />regularly to get this weed eliminated. This is not a problem at this time. (FINAL NOTE: No weed-infested aggregate <br />product must leave this permit area.) <br />(TS) Topsoil: <br />Narrative: There is topsoil stockpiled along the western edge of the pit. The operator must ensure that it is protected <br />against erosion and weed infestation. As additional areas are disturbed, the operator must ensure that topsoil from those <br />areas is also stripped and protected, for future use in reclamation. <br />PHOTOGRAPHS <br />(Placed after the contact addresses, below.) <br />Page 2 of 3
