PERMIT #: M-1985-085 <br />INSPECTOR'S INITIALS: RCO <br />INSPECTION DATE: September 7, 2010 <br />OBSERVATIONS <br />This was a monitoring and surety-related inspection, to verify that the site is being operated according to the approved <br />mining and reclamation plans, and to assess level of disturbance in relation to theamount of reclamation bond. The <br />operator has not filed a 2009 annual report or annual fee, and the Division has notified the operator that those items are <br />now due. If the required items are not submitted, the Division will proceed with enforcement action which may result in <br />the permit being revoked and the bond forfeited. The operator is encouraged to respond to those notices promptly. The <br />Division also attempted twice to contact the operator about this scheduled inspection, but was unsuccessful. The entrance <br />gate to the site was locked and the required permit ID sign was posted. <br />(AT) Acid And Toxic Materials: <br />Narrative: The site is clean and free of all debris, contaminants, or imported materials. <br />(BG) Backfilling and Grading: <br />Narrative: The site contains one pit, whose pit walls have been graded to 3:1 slopes. <br />(FN) Financial Warranty: <br />Narrative: The bond currently held by the Division is $25,000. The bond is adequate to reclaim all outstanding <br />disturbance. <br />(HB) Hydrologic Balance: <br />Narrative: There is no stormwater runoff and no exposed groundwater at the site. <br />(MP) Gen. Compliance With Mine Plan: <br />Narrative: This is approved as a phased operation, and the disturbance is limited to the phase areas in the central and <br />south portions of the site. Disturbance consists of a 3-acre pit, and another 2 acres of topsoil stockpiles S and NE of the <br />pit, pitrun and product stockpiles in the pit and W of the pit. Many of the piles on the site exhibit rabbitbrush plants, <br />indicating that they have been there for an extended time. By observation of loader and truck tracks, there appears to be <br />recent removal of some stockpiled material, and possibly some pit floor grading. <br />(OD) Off-site Damage: <br />Narrative: There is no offsite damage. <br />(RS) Reclamation Success: <br />Narrative: Other than grading the pit walls to final gradient, there is no other reclamation observed. <br />(RV) Revegetation: <br />Narrative: Disturbed areas have not been seeded, though some areas have been vegetated by volunteer native vegetation <br />(mainly rabbitbrush, blue grama, and sand dropseed).
