PERMIT #: M-1999-002 <br />INSPECTOR'S INITIALS: THM <br />INSPECTION DATE: August 17, 2010 <br />OBSERVATIONS <br />This inspection was conducted as part of the normal monitoring program established by the Colorado Division <br />of Reclamation, Mining and Safety (DRMS). Celina Akin with American Soda, LLP along with Paul Daggett <br />with the BLM-White River Field Office accompanied the inspector during the inspection. The American Soda <br />Yankee Gulch Minerals Project is located 22 miles southwest of Meeker, Colorado in Rio Blanco County. <br />The purpose of the site visit was to inspect the Mechanical Integrity Test (MIT). The operator is required to <br />conduct MIT every two years by the Evironmental Protection Agency to meet the requirements of the <br />operatotors Underground Injection Control Permit. The MIT was conducted by Mike Siepert of Alder Hot Oil <br />Service, Inc. and Daren Palmer of Tu & Frum Water. The test was conducted on both the long string and the <br />short string. During the test the wells were presurized above 300psi for a minimum of 30 minutes. A total of <br />17 wells were tested and only one failed, the 29-34 short string. <br />A location where the evaportion ponds were located and reclaimed was also observed during the inspection. <br />The success of the reclamation appeared to be successful with a variety of perennial grasses and shrubs <br />observed. All reclaimed slopes appeared to be no steeper than 3H:1V. <br />No noxious weeds were observed during the inspection. <br />The currently held bond amount of $236,424.00 is adequate at this time. <br />A copy of this ispection was supplied to Paul Dagget at the Bureau of Land Management - White River Field <br />Office.
