(Page 2) <br />MINE ID # OR PROSPECTING ID #: m-19Pn-944 <br />INSPECTION DATE: A/.3n/in <br />INSPECTORS INITIALS: -RK <br />OBSERVATIONS <br />The Division conducted a monitoring and construction progress inspection of the site on <br />8/30/10.Presnt during the inspection was Mr. Tim Comer, manager Environmental Resources. <br />The inspection began around the Phase IVC perimeter liner inter phase ditch area. The <br />west facing pads were being leached at the time of the inspection. The inter phase ditch <br />area was walked and inspected and found to be in good repair. <br />Continued the inspection to Phase IV pregnant pond area. During a routine maintenance of <br />the high volume pumps and replacement of a solution recovery pipe which required the <br />operator to shut off the ARD pumps, which in turn caused the Pressure transducer readings <br />for Phase IV low volume to be over the 2 feet threshold. The Division expects the <br />readings to exceed on temporary basis under such conditions. Pumps were back up within <br />hours and the readings were below the 24 inch threshold. <br />While in the area collected the following readings from the high and low volume solution <br />collection transducer for Phase I,II,III&IV. <br />Phase IV High Volume Solution Collection transducer # 7 was recorded at 44.1 # 8 was at <br />43.5 #9 was at 43.5 and # 10 was at 44.7. Low volume solution collection transducer # 1 <br />was recorded at5.8 and # 2 was at 8.7 inches of hydrostatic head. <br />Phase I high Volume solution Collection transducer # 1 was recorded at 25.4 # 2 was at <br />43.4 and # 3 was at41.1 piezometer for the pond was recorded at 50.1. Low volume solution <br />collection transducer # 1 was recorded at .34 and # 2 was at .47. <br />Phase II & III High Volume Solution Collection Transducer # 4 was recorded at 36.9 #5 was <br />at41.0 and # 6 was at 39.1. Low volume solution collection transducer # 1 was recorded <br />at.55 and # 2 was at .23 Piezometer for the pond was recorded at 41.7. Low volume <br />solution collection pressure transducer # 1 was recorded at .55 and # 2 was at .23 <br />Inspected the external pond it was holding about the same amount of water as noted during <br />the last inspection. Low volume solution collection transducer # 1 was recorded at 12.1 <br />and # 2 was at 10.6 inches of hydro static head. <br />Inspected the under drain discharge area. All lower ponds were dry. South under drain was <br />discharging about 16gpm and the 4 inch pipe was at a dribble at the time of the <br />inspection. AG1.5 was dry. <br />Inspected the Phase v leach pad construction area. Both clay liner and drain cover were <br />being deployed at the time of the inspection. The western portion of the pregnant pond <br />area is almost covered with drain cover material. Walked the entire length of the <br />pregnant pond to where the caisson and under drain discharge is located. The operator had <br />cut a 12 inch diameter hole to drain the water in this area. the division requested a <br />detailed picture sequence of the backfilling of this hole in the QA/ QC report with <br />pictures showing how the cement back fill was placed and the final repair work of the <br />liner . Mr. Tom Sheppard agreed to provide the details of the repair work in the QA.QC <br />report for the week. <br />Inspected the Cresson mine, it was active at the time of the inspection. The backfilling <br />of the East Cresson side should be completed by the end of the quarter if not sooner.
