PERMIT #: M-1995-060 <br />INSPECTOR'S INITIALS: RCO <br />INSPECTION DATE: August 11, 2010 <br />OBSERVATIONS <br />This was a monitoring inspection to see whether there have been any changes in the condition of the site. The annual <br />reports do not reflect any reported activities in several years. <br />(BG) Backfllling and Grading: <br />Narrative: There has been about 10 acres affected by past excavation, but no backfilling or grading performed to-date. <br />Excavation disturbance has resulted in several low areas that do not drain and tend to impound water. <br />(FN) Financial Warranty: <br />Narrative: The bond for this permit is $61,370 and was calculated to cover the reclamation of the entire disturbed site. <br />Since only about 25% of the site is disturbed and the site is and has been inactive for several years, the bond is considered <br />to be sufficient. When operations recommence the office will recalculate the bond amount. <br />(HB) Hydrologic Balance: <br />Narrative: The several areas of ponded water were along the eastern edge of the site, and appeared only a few inches <br />deep. Excavation of material there has only been about three feet deep, but the clay in the mined aggregate or underlying <br />it probably contributes to the inability of the water to percolate easily into the ground. It is felt that the water is not <br />exposed groundwater, but is due to precipitation. Since the operator does not have approval for a water surface, the <br />operator should backfill some of the native material from higher, unmined areas onto the low, non-draining areas. The <br />uppermost layer of unmined material on the site appears more sandy and well-drained. <br />(MP) Gen. Compliance With Mine Plan: <br />Narrative: The operator is generally in compliance with the approved plan. See comments under "reclamation" and <br />"other." <br />(OR) Other: <br />Narrative: The site is located adjacent to a county road at the edge of the town of Alamosa. The site has been a long-time <br />location for casual, unauthorized dumping of trash. Even though the operator may not find it feasible to fence such <br />visitors out, or to otherwise restrict this activity, it is nevertheless not an approved activity. The operator must monitor the <br />site to ensure that no hazardous contaminants are dumped, and to remove the dumped materials occasionally. The cost of <br />removal and disposal of the dumped items will be added to the reclamation liability. <br />(RS) Reclamation Success: <br />Narrative: There has been no reclamation performed to-date. However, the operator could perform some backfilling of <br />the low, non-draining areas to eliminate the ponding and to begin revegetating the areas that will not be redisturbed again. <br />(RV) Revegetation: <br />Narrative: There has been no revegetation performed to-date. No noxious weeds were observed. <br />(SC) Sediment Control: <br />Narrative: The surrounding land is undulating and irregular, but no stormwater leaves the site and all sediment remains <br />onsite. <br />(SM) Signs and Markers: <br />Narrative: The operator has marked the corners of the site in the past, but the markers are no longer visible. The operator <br />must re-install permit boundary markers, especially along the eastern side, where excavation disturbance is located and <br />where more dumped trash exists on- and offsite. <br />(TS) Topsoil: <br />Narrative: There is no well-defined topsoil layer. There is a little surface material stripped and piled along the southern <br />edge of the disturbed area, presumably to be replaced during reclamation. If no topsoil is required to be replaced, then <br />this berm of material may be available for backfilling low areas. <br />Page 2 of 3
