PERMIT #: M-1990-059 <br />INSPECTOR'S INITIALS: RCO <br />INSPECTION DATE: August 15, 2010 <br />OBSERVATIONS <br />This was a routine monitoring inspection that was performed while this inspector was in the area to check other sites <br />nearby. The site has not been active, and the operator was not contacted. <br />The site has been inactive long enough that it should be reactivated or the last steps of final reclamation should be <br />commenced. Please refer to Hard Rock/Metals Rule 1. 13, concerning declaration of temporary cessation, five-year time <br />limit on temporary cessation, and resumption of activity at end of period of temporary cessation. <br />(AT) Acid And Toxic Materials: <br />Narrative: There was no iron-stained material or seepage visible on the dump surface, no ore or pyrite on the site, and no <br />chemicals or contaminants present. <br />(BG) Backfilling and Grading: <br />Narrative: The site has been in reclamation for several years. Backfilling of the portal was performed, though it appears <br />that it settled or was re-opened to a small extent, leaving a 2-foot wide opening. This opening does not meet the <br />reclamation plan, and additional backfilling work is needed. The dump was recontoured, and the cut face above the portal <br />was filled and sloped. These surfaces exhibit good slope gradients and no settling or erosion. <br />(FN) Financial Warranty: <br />Narrative: The bond amount is $1,000. It is sufficient to perform the remaining reclamation tasks. <br />(HB) Hydrologic Balance: <br />Narrative: The site is dry, with no discharge, ponding or stained runoff. <br />(RD) Roads: <br />Narrative: The portion of the access road south of the creek (furthest from the county road) was barricaded and seeded. <br />(RS) Reclamation Success: <br />Narrative: The regraded surfaces of the disturbed area (excluding the portal opening itself) appear to be adequately <br />reclaimed, making the small area of disturbance difficult to discern from surrounding areas. <br />(RV) Revegetation: <br />Narrative: The site is well vegetated with grass and forb cover.
