(Page 2) <br />MINE ID # OR PROSPECTING ID #: M-1997-054 <br />INSPECTION DATE: Z/ii1n INSPECTORS INITIALS: -RK- <br />OBSERVATIONS <br />The Division conducted a monitoring inspection of the site on 7/31/10, as part of sites <br />to be inspected during the fiscal years 2010-2011. The site is a 513.2 acres permit area <br />consisting of a sand and gravel operation along the south west side of the permit and a <br />granite quarry along the north east side of the permit area. The maximum acres that can <br />be affected at any given time are 200 acres. Warranty for the site is $1,174,724.00 <br />Present during the inspection was Mr. Terry Tew, the plant manager for the operation. <br />Permit sign, boundary and affected area markers were properly posted. <br />The entire area was driven walked and inspected. With the exception of routine <br />maintenance like the settling pond cleaning and other mining area routine, the site was <br />not actively producing aggregate. <br />The operator had cleaned up most of the non mine related waste material that was noted <br />during the last per operational inspection in 2007, when an amendment to the site was <br />submitted. <br />Large quantities of processed material from the sand and gravel portion of the operation <br />were stockpiled along the southeast side of the pit area. In anticipation of mining, top <br />soil/overburden has been removed and temporarily stockpiled along the west side of the <br />permit area. Operator plans to utilize the topsoil/overburden during their anticipated <br />reclamation of the area. If the top sol/overburden is not utilized within 180 days, Mr. <br />Tew stated that it will be seeded and stabilized in place. No erosion features were noted <br />in the already stockpiled material. <br />The operator has placed the large oil storage tank in a properly fenced and appropriately <br />lined area to protect from spills. The entire minor clean up request that were mentioned <br />in the previous inspection report for the most part has been complied with. <br />The active SDG mining area was dry at the time of the inspection. No shallow GW was <br />exposed at the time of the inspection. Operator has a SEO approved adequate water <br />substitute supply plan for the operation. The sand and gravel requires washing to remove <br />fines. Te large old processed material with lots of fine along the South East side of the <br />active mining and stockpiling area was still in place. Side slopes stable. Mr. TEW said, <br />Front range aggregates plans to find use for it soon, once the current market condition <br />changes. All of the material from the site is hauled by rail that is owned by the <br />operator. The storm water control structures for the site were found to be in good <br />repair. <br />As stated earlier, an excavator was cleaning the east side settling pond at the time of <br />the inspection. Three settling ponds and a fresh water pond are utilized in the <br />processing of the aggregate from the sand and gravel side of the operation. <br />The north side f the permits traversed by the Tallahassee Creek. Both the west side <br />bridge and the East side crossing had been washed out. The West side bridge was washed <br />out during the past rain storm in 2010. The east side crossing to the granite quarry is <br />was washed out in 1996. Since the operator did not affect any of the areas amended in <br />2007 associated with the granite quarry operation, the Division did not see the need to <br />inspect the area. Mr. Tew is aware of the stipulation associated with the granite quarry <br />and assured the Division that if and when the operation starts mining the quarry, the <br />requested information will be provided that will fulfill the amendment approval for the <br />site. The operation is in compliance with the approved permit. No warranty calculation is <br />needed at this time because the operation has not even started in the quarry area. <br />I & E Contact Address <br />NAME: MR.
