I was not accompanied by an operator contact, but checked in at the mine office and spoke to Dennis
Jones, SCC Hydrologist, prior to the inspection. Weather was cloudy, with intermittent rain showers that began
shortly after starting the inspection. The inspection was limited to portions of the mine site accessible from the
Sage Creek County Road. I parked along the road adjacent to Sediment Ponds 012,013, and 014, walked up to
each of the pond embankments, and viewed the reclaimed watersheds from the vantage point of each embankment.
HYDROLOGIC BALANCE -Rule 4.05❑El Drainage Control 4.05.1, 4.05.2,4.05.3; Siltation Structures 4.05.5,
4.05.6; Discharge Structures 4.05.7, 4.05.10; Diversions 4.05.4; Effluent Limits 4.05.2; Ground Water
Monitoring 4.05.13; Surface Water Monitoring 4.05.13; Drainage—Acid and Toxic Materials 4.05.8;
Impoundments 4.05.6,4.05.9; Stream Buffer Zones 4.05.18:
Ponds 012,013, and 014 were inspected. Embankments appeared to be stable and no problems were
observed. Pond 012 was discharging clear flow through the dewatering orifice in the spillway riser. Pond 013
water level was approximately 1 foot below the top of the spillway riser; no discharge. Pond 014 water level was
approximately 18"below the top of the spillway riser; no discharge.
SLIDES and DAMAGE -Rule 4.12:
In the Pond 012 lower watershed, remediation of the"Pond 012 Slide"is continuing. At the time of
inspection, a scraper, loader, and truck were excavating material from along the lower portion of the slide area.
The slope has been cut back to what appears to be competent shale strata along the length of the former scarp,
from a point approximately 20 yards NE of the Pond 12 embankment,to the brush-line east of the slide.
Excavated material was being hauled to a couple locations along the temporary road to the east of the slide, for use
as backfill during road reclamation.
Number of Partial Inspection this Fiscal Year: 1
Number of Complete Inspections this Fiscal Year: 0
Page 2 of 5
