THE AREA WHERE WORK HAS BEEN PREFORMED IS AN OLD <br />MINED OUT AREA. THE AREA WAS SHRINK STOPED OUT. ABOVE THIS AREA WHERE LOOSE <br />GROUND IS ON HANGING AND FOOT WALLS OF THE OLD STOPE. THERE ARE VISIBLE STULLS <br />IN THE OLD STOPS. INFORMATION FROM THE FOREMAN WAS THAT LOOKING AT OLD MAPS OF <br />THE MINE THE BACK COULD EASILY BE 250 FEET UP FROM THE MINE FLOOR. WITH A MINE <br />LIGHT YOU COULD NOT SEE ALL THE WAY TO THE TOP. GROUND SUPPORT IS NECESSARY <br />AND SHALL BE DESIGNED, INSTALLED AND MAINTAINED TO CONTROL THE GROUND WHERE <br />PERSONS WORK OR TRAVEL IN PERFORMING THERE ASSIGNED TASK. <br />THIS CONDITION WAS A FACTOR THAT CONTRIBUTED TO THE ISSUANCE OF IMMINENT <br />See Continuation Form (MSHA Form 7000-3a) <br />9.
