PERMIT #: M-1981-022 <br />INSPECTOR'S INITIALS: MAC <br />INSPECTION DATE: July 28, 2010 <br />OBSERVATIONS <br />The inspection was conducted by Michael Cunningham of the Division of Reclamation, Mining and Safety (Division). <br />John Warren of Connell Resources, Inc. was also present for the inspection. The Glass Gravel Pit is located 1/2 mile north <br />of Fort Collins, Colorado. The site is permitted for 15.4 acres and post-mining land use is recreation. <br />(FN) Financial Warrant <br />The Division holds a financial warranty in the amount of $148,500.00, which has been determined to be sufficient to <br />complete reclamation. <br />(BB) Hydrologic Balance: <br />The groundwater pond has not been lined. The Division has a copy of the Operator's well permit and temporary <br />substitute water supply plan, which expires on October 31, 2010. <br />(RS) Reclamation Success: <br />A pile of concrete cinder blocks, metal pipes and other refuse was observed on the west side of the groundwater pond. <br />According to the Operator, this material was illegally dumped on the site. The presence of this material is considered a <br />problem and will require corrective action by the Operator. <br />The Operator informed the Division over five years ago that mining is complete. Rule 3.1.3 of the Construction Materials <br />Rules requires reclamation to completed within five years from the date the Operator informs the Division mining is <br />complete. Amendment - 02 (AM-02), approved by the Division on May 18, 2005, changed the reclamation plan to allow <br />for the removal of the groundwater pond. The current reclamation plan states the groundwater pond will be removed and a <br />4 acre, clay lined depression will be left in its place. The Operator indicated the City of Fort Collins is interested in <br />acquiring the site and may want to retain the groundwater pond. The Operator should be aware that any change to the <br />reclamation plan will require the submittal of a technical revision or amendment to the permit. <br />(RV) Reve etation: <br />Narrative: Several Russian Olive trees were noted along the south shore line of the groundwater pond. Russian Olive is <br />listed on the Colorado Noxious Weed List; the presence of this noxious weed is considered a problem and will require <br />corrective action by the Operator. <br />This concluded the inspection. <br />Page 2 of 4
