OPERATORS <br />SHOULD READ THE FOLLOWING PAGES CAREFULLY IN ORDER TO ASSURE COMPLIANCE <br />WITH THE TERMS OF THE PERMIT AND APPLICABLE RULES AND REGULATIONS. If a <br />Possible Violation is indicated, you will be notified under separate cover as to when the Mined Land <br />Reclamation Board will consider possible enforcement action. <br />INSPECTION TOPIC: Availability Of Records <br />PROBLEM/POSSIBLE VIOLATION: Problem: The Division does not have a record of a Mining Plan Map or a Reclamation Plan <br />Map. Rule 6..3.5(1) of the Hard Rock/Metal Mining Rules and Regulations requires the Operator to provide a map that clearly describes <br />the features associated with the mining plan and the components of the Reclamation Plan. <br />CORRECTIVE ACTIONS: The Operator shall submit one map for the mine plan and one map for the Reclamation Plan. The maps <br />must be drawn to scale no smaller than appropriate to clearly show all elements that are required to be delineated by the Act and the <br />Rules; show a north arrow, note any section corners adjacent to the operation, and indicate the date illustrated. At a minimum, the maps <br />must include the information outlined in Rule 6.2.1(2), 6.3.5(2) and 6.3.5(3). The maps shall be submitted to the Division in the form of <br />a Technical Revision along with a revision fee of $216.00. The maps shall be submitted to the Division on or before the corrective <br />action date. <br />CORRECTIVE ACTION DUE DATE: 8/27/10 <br />INSPECTION TOPIC: Signs & Markers <br />PROBLEM/POSSIBLE VIOLATION: Problem: The affected area boundary markers are missing or incorrectly placed. This is a <br />problem for failure to maintain boundary markers around the affected area as required by Section 3.1.12(2) of the rule. <br />CORRECTIVE ACTIONS: The operator shall conduct a survey and replace the boundary markers in the correct location(s). The <br />operator shall provide proof to the Division that this has been done by the corrective action date.
