• Hard Rock/Metal Mining Permit Application <br />Limited Impact Designated Mining Operation (I 10(d)) <br />Venture Resources, Inc. - Hukill Gulch Millsite <br />Revision 1: July 27, 2010 <br />Exhibit F <br />Venture Resources, Inc. hereby acknowledges the following in terns of additional permits, licenses, permissions, <br />etc. necessary from various jurisdictional authority: <br />Effluent Discharge Permit - n/a, there is no process discharge from this proposed mining operation. <br />Stormwater Permit - a CO Water Quality Control Division Stormwater Permit will be required. See application <br />attached as an appendix to tlus Exhibit F. <br />Air Quality Emissions Permit - n/a, emissions are well underneath the CDPHE APEN thresholds. <br />Radioactive Source Mat'l License - n/a <br />Disposal of Dredge Mat'l Permit - n/a <br />Permit to Construct a Dam - n/a, proposed Concrete Leachate Collection barrier is an exempt structure according to <br />Co Div. of Water Resources. <br />Well Permit - monitoring/sampling wells will require CO Div. Water Resources permit. See issued permits <br />attached as an appendix to this Exhibit F. <br />• Explosives Permit - it is acknowledged that our operations will require this permit/license if explosives are used. <br />State Historic Preservation Office clearance - n/a <br />Highway Access Permit - n/a <br />US Forest Service Permits - n/a, privately owned land and rights-of-way and not within USFS boundaries. <br />BLM Permits - n/a, privately owned land and rights-of-way <br />County Zoning and Land Use Approval - n/a, this use is in strict conformance with Clear Creek County Zoning, <br />Section 7 - Mining Districts, M-I and M-2 zoning designation. <br />County Building Permit - it is acknowledged that any structures greater than 120 sq. ft. will require a building <br />permit. <br />County Road & Bridge Permit - it may be necessary to apply for a maintenance permit prior to performing any road <br />maintenance activities. <br />CDPHE Letter of Intent - it is acknowledged that a Letter of Intent may be necessary from the CDPHE to land apply <br />Class A biosolids as part of the reclamation plan. <br />US Army Corps of Engineers Permit - n/a, this is not an aquatic site. See HP Geotech's aquatic site evaluation <br />attached as an appendix to this Exhibit F. <br />• Mine Safety and Health Administration - it is acknowledged that this operation will regulated by MSHA. <br />Page 1 of 2
