(Page 2) <br />MINE ID # OR PROSPECTING ID #: m-19Rn-9aa <br />INSPECTION DATE: 6./9-!10 INSPECTOR?S INITIALS: -RK <br />OBSERVATIONS <br />The Division conducted a monitoring and construction progress of the Phase V leach pad <br />construction on 6/22/10. Present during the inspection were, Mr. Mark Vanoni and Mr.Timm <br />Comer of CC&V. <br />Inspected Phases I,II,III&IV, of the leach pads. The inter phase perimeter lined ditch <br />along Phase IV C was found to be in good repair. Phase IV pad was being actively loaded <br />with ore at the time of the inspection. <br />No stability of the pad was noted along the bore holes for the hydro jex metal recovery <br />system. <br />While in the area, collected the following readings from the High and Low Volume Solution <br />collection pressure transducers. <br />Phase I High Volume Solution Collection Transducer # 1 was recorded at 24.5 #2 was at <br />35.7 and # 3 as at 38.7. Low volume transducer # 1 was at .37 and # 2 was at .43. <br />Piezometer for the pond was at 45.5 <br />Phase II and III High Volume transducer # 4 was recorded at 14.8 # 5 was at 14.0 and #6 <br />was at 13.6. Low Volume solution collection transducer # 1 was recorded at .48 and # 2 <br />was at 0.13. Pizometer reading for the pond was recorded at 29.1 <br />Phase IV High Volume Solution Collection Transducer # 7 was recorded at 12.6 and # 8 was <br />at 14.0 # 9 was at 10.6 and # 10 was at 12.7.Low volume solution collection transducer # <br />1 was recorded at 12.6 and # 2 was at 10.1. <br />Inspected the under drain area. Both the south and four inch pipe were discharging, with <br />much diminished flow. <br />Inspected the Phase V leach pad construction area. Deployed liner was walked and <br />inspected and was found to be in good repair. Leak collection gravel was being placed in <br />the west and south side of the pregnant pond while geomebarne was being deployed along <br />the east and north side of the pond. The leach pad sub grade preparation along the North <br />side was also continuing. Amec QA/QC personnel were on site verifying the deployment at <br />the time of the inspection. <br />I & E Contact Address <br />NAME: MR.
