PERMIT #: M-1981-286 <br />INSPECTOR'S INITIALS: MAC <br />INSPECTION DATE: July 8, 2010 <br />OBSERVATIONS <br />The inspection was conducted by Michael Cunningham of the Division of Reclamation, Mining and Safety (Division). <br />Richard Thomas and Ray Thomas of RWT Excavating and Demolition were also present for the inspection. The Thomas <br />Gravel Pit 4 is located approximately 4.5 miles west of Keota, Colorado. The site is permitted for 87.60 acres and the <br />approved post mining land use is general agriculture. <br />This inspection was conducted in response to the submittal of a Transfer of Permit and Succession of Operators <br />Application, which was received by the Division on June 25, 2010. The Thomas Gravel Pit 4 is currently operated by <br />Weld County Public Works Department; the successor Operator is RWT Excavating and Demolition, LLC. The successor <br />Operator chose to waive their right to a 30-day processing time to allow the Division to recalculate the Financial Warranty <br />requirements. The purpose of this inspection was to determine the amount of financial warranty the Division must hold in <br />order to complete reclamation. <br />The permit sign was posted at the entrance to the site and the boundary of the affected area was clearly delineated per <br />Rule 3.1.12. During the last inspection conducted on September 14, 2010, it was noted mining operations were complete <br />and reclamation activities were underway. At the time of this inspection, all reclamation has been completed. The access <br />road was reclaimed foll9wing the previous inspection; vegetation is starting to establish on the road. No disturbance has <br />occurred since the previous inspection. <br />The Division has calculated the financial warranty based on a maximum allowed disturbance of 10 acres. The amount of <br />the Financial Warranty was determined to be $23,619.00.
