Pirneda <br />Director <br />Re: Environmental Protection Plan for DMO Amendment Materials, Permit Number M-1997-032, Van 4 Mine, <br />Amendment AM-1; Request for Operator's acceptance of Amendment Requirements, for Adequacy of <br />Amendment. - - - <br />Dear Ms. Woodward, <br />The Division has completed its review of the most recent adequacy submittal, pertaining to the EPP that is being <br />finalized for the Van 4 Mine permit. The permit is considered to be a DMO, and as such, is being amended to include <br />the EPP. This letter describes the Division's acceptance of details of Denison's proposed plans, and it lists specific <br />details for which the Division still requires Denison's written acceptance, toward gaining approval of the amendment. <br />Ore Pad Liner. <br />The Division accepts the following aspects of the ore pad liner: the location upon the mine pad; the dimensions of the <br />ore pad; the chosen PVC liner material; the ore pad design, including bedding and cover materials; and the <br />installation specifications and certification. <br />The geotextile liner material to be used will be PVC as detailed in Denison's proposal. The plan proposes that the liner <br />will be manufactured and shipped to the site as a one-piece liner to install. If the liner is a one-piece unit, the <br />presumption is that all seaming and testing will be performed by the manufacturer at the manufacturing facility, and <br />the Division accepts that there is no need for seaming at the site at the time of installation. However, in lieu of onsite <br />seam testing, the Division will require copies of the manufacturer's tests for the liner. Please provide written <br />acceptance of this requirement. <br />Earthen sub-grade and cover materials to be utilized will be obtained onsite, and handled and processed according to <br />Denison's proposal. Follow-up certification reports that include all aspects of the installation, must be provided to the <br />Division, for each liner installed. Please provide written acceptance of this requirement. <br />All site preparation and installation must proceed according to the approved plan, and overseen and documented by <br />qualified onsite QC monitor, who, per Hard Rock/Metals Rules, must be a Colorado registered PE. The installation of <br />the liner will be performed and overseen by qualified personnel, as described by the manufacturer and also proposed <br />by Denison. This will include deploying under proper conditions of ambient temperature, wind, soil moisture, <br />precipitation, proper trenching and anchoring, and performing the necessary inspections throughout the process. <br />Please provide written acceptance of this requirement. <br />Written Acceptance of Amendment Requirements and Conditions. All technical adequacy issues that were reviewed <br />by this office and previously found adequate are not discussed herein.
