(Page 2) <br />MINE ID # OR PROSPECTING ID #: M-1981-080 <br />INSPECTION DATE: 6.7/1 Q INSPECTORS INITIALS: -BK- <br />OBSERVATIONS <br />The Division conducted an inspection of the site as part of sites to be inspected during <br />the fiscal years 2009-2010. The site is a small 9.9 acres, 110 sand and gravel operation <br />located approximately 3 miles west of Texas Creek along the south side of Highway 50. <br />The site was not active at the time of the inspection. Present during the site visit were <br />Mr. Mike Tezak, the operator. <br />Permit sign and boundary markers were properly posted. Because of lack of demand the only <br />mining related activity on site is removal of some material as needed from already <br />stockpiled processed, Approximately 4 acres is disturbed including the small top soil <br />stockpile area along the South West side of the permit area. <br />The areas along the west and north side of the permit which are graded to approximate <br />slopes of 3:1 or gentler appeared stable. The total affected area is mainly along the <br />north east side of the permit area with a small high wallapproximately5 feet high and 30 <br />feet long. Most of the permit area long the west side had been graded to almost 5:1 or <br />gentler slope. Under Division approved TR, in 2000, operator is allowed to have an active <br />Asphalt Batch plant. There was no asphalt batch plant noted at the time of the <br />inspection.
