Carter <br />Nuvemco, LLC 43 CFR 3809 - Surface Management <br />10771 3200 Road Mining Plan of Operations <br />Hotchkiss, Colorado 81419 <br />DECISION <br />LAST CHANCE MINING PLAN OF OPERATIONS APPROVAL <br />Dear Mr. Carter: <br />/il - -coo s - o0- <br />Thank you for the mining plan of operations (POO) submitted to conduct operations in portions of <br />T. 46 N., R. 18 W., Section 12, NW'/4, N.M.P.M., as required by the Surface Management Regulations <br />found at 43 CFR 3809. On November 4, 2008, we received your POO for underground bulk sampling <br />and exploration drilling activities to be conducted at the Last Chance project area; it has the assigned <br />BLM serial number COC-71183. An Environmental Assessment (EA) has been completed, and the <br />mining POO is hereby approved. You may begin operations with the following conditions: <br />o Nuvemco is authorized to operate the Last Chance Mine Project as defined in the Final mining <br />POO, dated June 2010, which includes continuing to conduct underground exploration mining, <br />and to drill up to 358 shallow exploration drill holes using air drilling methods. Total bulk <br />exploration and feasibility sampling is limited to removal of less than 1,000 tons of presumed <br />ore. The operator will provide this office with weight receipis for ail ore removed from the <br />project area within 30 days of ore removal. <br />o Nuvemco is required by Colorado State Law and Regulations to have a permit issued by the <br />Colorado Division of Reclamation, Mining and Safety (CDRMS). Operations may begin when <br />the CDRMS permit is issued and the bond is filed and accepted by CDRMS. A copy of the <br />bond is required to be submitted to this office. <br />o Nuvemco is required to get all other Federal, State, and local government authorizations, as <br />necessary. This includes, but is not limited to, Montrose County approval for all county road <br />use and improvements. A copy of the permit is required to be submitted to this office. <br />o Nuvemco is required to control weeds for the life of the POO, until final BLM and State permit <br />closure, in accordance with this office's weed management strategy. The proposed project area <br />will be re-surveyed for invasive, non-native plant species prior to commencement of the <br />proposed operations and the results of this survey will be copied to this office.
