(Page 5) <br />MINE ID # OR PROSPECTING ID # M-1980-041 <br />INSPECTION DATE 06/18/2010 <br />INSPECTOR'S INITIALS PSH <br />The following problems (PB) and/or possible violations (PV) (and suggested corrective actions) were identified <br />during this inspection. The problems should be corrected by the dates given, or they will become possible <br />violations. The possible violations should be corrected by the dates given to reduce their severity when <br />considered by the Mined Land Reclamation Board. The inspector noted on the previous page should be <br />notified of all corrective actions taken. <br />PROBLEMS/POSAIBLE VIOLATIONS <br />AND <br />CORRECTIVE ACTIONS <br />PB or PV: PV <br />TOPICS: MP, OD <br />CORRECTIVE DATE: March 15, 2011 <br />PROBLEM <br />The Operator/landowner failed to first obtain from the Board or Office a reclamation permit before engaging <br />in a new operation in accordance with A4-32.5-109(1) of the Colorado Land Reclamation Act for the Extraction <br />of Construction Materials. <br />CORRECTIVE ACTIONS: <br />1. Cease all mining related activity at the site immediately upon receipt of this inspection report except for <br />those activities required to prevent damage to off-site areas or protect the public health and safety. <br />2. Obtain an 112c Construction Materials reclamation permit as described in the consent agreement mailed <br />under separate cover between the Division and Operator and/or as required by the Colorado Mined Land <br />Reclamation Board's signed Board order.
