At the time of this <br />application, a Mine Safety and Health Administration identification number has <br />not been obtained. <br />2.03.5 Compliance Information <br />Neither the applicant nor any subsidiary, affiliate or person controlled by or <br />under the common control of Northfield Partners LLC, has had a Federal nor <br />State minima permit suspended or revoked. Likewise, the applicant and <br />subsidiary, affiliate or person controlled by or under common control with <br />Northfield has not forfeited any mining bond or similar security deposited in <br />lieu of bond. Since no suspension, revocation or forfeiture has occurred, <br />Section 2.03.5(2) is not applicable. <br />Neither the applicant nor any subsidiary, affiliate or person controlled by or <br />under the common control of Northfield Partners LLC, has received a violation <br />notice during the three year period preceding the submission of this <br />. application. <br />Upon notification of approval of this application but prior to permit issuance, <br />Northfield Partners LLC will provide any applicable update, <br />2.03 -7 Revised 3/3/2008 <br />
