Powertech (USA) Inc. (Powertech) hereby requests approval from the Colorado Division of Reclamation
and Mining Safety (DRMS) to conduct baseline environmental site characterization activities, results of
which will be utilized in various permit applications to conduct mining operations.
Included within this package, DRMS will find completed Forms 1 and 2 providing Notice of Intent to
Conduct Prospecting Operations for Hard Rock/Metal Mines, along with a check for the $86.00
application fee. Additionally, please find the Addendum and Attachments listed below, which describe
the proposed baseline environmental site characterization activities.

• Addendum 1- Description of roving gamma survey and soil sampling activities.
Attachments:
• Attachment A - Tabulated locations of second phase roving gamma survey and soil sampling
