Revisions to Permit <br />Revisions made to the permit since the last permit renewal (RN-05, approved. on 9/10/2008) are <br />listed below. <br />Bond Releases <br />SL-08, Approved 1/05/2009. Phase I bond release of 476.6 acres. Released liability of $0.00. <br />SL-09, Approved 12/2/2008. Phase II bond release of 294.6 acres. Released liability of <br />$898,333.00. <br />SL-10, Approved 11/17/2008. Phase III bond release of 305.0 acres. Released liability of <br />$228,664.00. <br />Permit Revisions <br />PR-06, Approved 9/21/2009. Mine plan, including placement of overburden spoil material <br />from the K Pit into Horse Gulch Fill and K Pit Buttress Fill. <br />Technical Revisions <br />TR-103, Approved 8/18/2008. Mining and Reclamation Plan update in A, K:, and Z Pits. <br />Update of Deal and Horse Gulch pond information. <br />TR-104, Approved June 27, 2009. 2008 ARR. <br />TR-105, Approved September 14, 2009. Topsoil stripping in Horse Gulch area. <br />TR-106, Approved May 27, 2010. Revision to mining sequence in K Pit, including removal of <br />K Pit Buttress Fill from the plan. <br />TR-107, Approved May 27, 2010. 2009 ARR. <br />In TR-106, the mine plan was revised to alter the mining sequence in K Pit, including removal of <br />K Pit Buttress Fill from the plan. Other revisions include alterations to roads that will be mined <br />through and a new slide stability monitoring system. This change will be reflected in the permit, <br />namely in Sections 3.1.4.1 (Mining Sequence), 3.3 (Drilling), 3.5 (Backfilling and Grading), and <br />4.8 (Hydrologic Impacts). Several maps and Appendix T (Buttress Fill Geotechnical report) will <br />also be revised. <br />Minor Revisions - None after July 2008 <br />Violations <br />No Notices of Violation have been issued to the permittee since the last pen-nit renewal. <br />Permit Stipulations <br />There are no Stipulations attached to the permit. All previous stipulations have been complied <br />with or incorporated into the Permit Application Package as permit commitrnents during previous <br />Trapper Midterm 2010 Findings.REV 3.docx Page 4
