(Page 2) <br />MINE ID # OR PROSPECTING ID #: m-19An tad <br />INSPECTION DATE: 519611 n INSPECTORS INITIALS: _RK <br />OBSERVATIONS <br />The Division conducted a monitoring INSPECTION OF THE Phase I pregnant pond auxiliary <br />caisson drilling process as approved in Technical Revision 58, while in the area <br />conducting other inspections. Present during the inspection was Mr. Mark Vanoni, of CC&V. <br />The operator CC&V had requested and obtained an approval to place an additional caisson, <br />in the Phase I Pregnant pond to be utilized as needed for additional pumping of solution <br />from the pond. The drilling required a contractor which had previous drilling experiences <br />in such projects to within 10 feet of the upper geomembrane liner. <br />The Division wanted to make sure there was no damage to the liner, by verifying the <br />readings from the Low Volume Solution Collection Transducers, that collect solution from <br />the allowable leak from the upper liner and pumps it back into the pond. The Low Volume <br />solution collection transducer has a maximum of 2.0 feet of head allowed in a double <br />lined system. <br />At the time of the inspection, all the drilling had been completed and the caisson in <br />place. Drillers were pumping the drilling mud out of the casing walls that was utilized <br />to stabilize it in place. <br />The Division briefly talked to the Drilling crew and verified that the drilling was <br />conducted without any problems. <br />The Division collected the following readings from the High and Low Volume Solution <br />Collection transducers for the pond. <br />High Volume solution collection transducer # 1 was recorded at. 45.8 # 2 was at 33.6 and # <br />3 was at37.6.
