(Page 2) <br />MINE ID # OR PROSPECTING ID #: M-1987-103 <br />INSPECTION DATE: 4/28/2010 <br />INSPECTOR'S INITIALS THM <br />OBSERVATIONS <br />This inspection was conducted as part of the normal monitoring program established by the Colorado Division of <br />Reclamation, Mining and Safety. The East Douglas Pit is located approximately 21 miles south of Rangely, <br />Colorado in Rio Blanco County off of Rio Blanco County Road 27. The site is a 110c with 7.4 acres included in the <br />permit area. <br />The mine identification sign was in place but it was not legible, therefore not in compliance with Rule 3.1.12. The <br />sign is located on the fence at the main entrance on the north side of the gravel mine. The sign not being legible is <br />seen as a problem and must be repaired and photo-documentation of the repaired sign must be submitted to <br />this office by the date indicated on the last page of this report. <br />The site was active during the inspection. The majority of the material extraction was being conducted along the <br />west highwall. A wheeled loader was observed along with many stockpiles throughout the site. <br />A fuel tank was observed without secondary containment at the southernmost corner of the permit area. Per Rule <br />3.1.6, measures to minimize disturbance to the prevailing hydrologic balance of the affected land and surrounding <br />land and to the quantity and quality of surface and groundwater must be carried out by the operator. Lack of <br />secondary containment is considered a problem. The operator must construct secondary containment for the <br />fuel tank no later than the date indicated on the last page of this report. <br />In an effort to ensure the financial warranty remains adequate to reclaim this site per the requirements of the <br />approved reclamation plan, DRMS will update the reclamation cost estimate.
