IQ road, and b) construction of <br />drill pad access roads. The drill pads and their associated access roads will be authorized under a <br />Special Use Permit (SUP), while a Road Use Permit (RUP) will be issued for authorizing use on <br />NFSRs. You shall perform maintenance, or.deposit funds sufficient to cover the cost of <br />maintenance, commensurate with your use of NFSR roads authorized by the RUP. <br />In order to issue a Road Use Permit for authorizing use of the two NFSRs during the core drilling <br />program, The Forest Service needs the following additional information: <br />1. Please complete Page 1 of the enclosed "Application for Road Use Permit" form. Be sure <br />to indicate the Critical Vehicle dimensions (widest vehicle dimension and longest vehicle <br />dimension) under question #2. Instead of filling out question #4, please complete the <br />enclosed "Summary of Proposed Road Use "form (discussed in more detail below). <br />2. Fill out the "Breakdown of traffic" tables. There are 3 tables representing each phase of <br />the project: <br />a) traf c resulting Flom road improvements (i.e. to t..c NFSRs) or from <br />construction activity (i.e. drill pad and associated access road improvements), <br />b) commercial road use operations - the amount of traffic on the NFSRs that <br />would occur during project work (i.e. during core drilling operations), and <br />c) post operations and maintenance - the amount of traffic on NFSR's that would <br />occur post construction for maintenance and monitoring of the facility or project. <br />(i.e. reclamation of the access roads and drill pads, continuing commensurate <br />share of road maintenance on NFSRs). <br />?? <br />Caring for the Land and Serving People Printed on Recycled Paper
