Martin <br />Executive Director <br />The Division's Denver office received the above-named permit amendment application on May 3, 2010, when it was <br />then routed to our Durango field office for an initial completeness review prior to a technical adequacy review. Our <br />completeness review reveals that there are a couple items that must be provided, and until they are received the <br />amendment cannot be considered complete and the Division's technical review may not begin. Please provide or <br />revise, therefore, the following items: <br />Application Form <br />Blanks number one, six and seven, on page one of the application, identify the applicant/operator and owner as "Mount <br />Sneffels Mining Co. LLC". This is also how the applicant/operator is identified on page two, and how it was signed on <br />page seven. Please note that though it is consistent throughout the application, this does not perfectly match the <br />operator's full name, as approved. <br />In the Succession of Operator SO-01, which was approved in 2007, the operator's name is documented to be slightly <br />different, as "Mount Sneffels Mining Company, LLC". The Colorado. Secretary of State's Business Section lists <br />"Mount Sneffels Mining Company, LLC" as a licensed entity in good standing. This is considered to be the correct <br />and complete name, and this is exactly how it should appear in all amendment application documents (including <br />punctuation). <br />On page seven, the blank for the date above the signature line, should also be filled in when this is re-signed and re- <br />notarized. <br />Please resubmit the application form with all references to the operator corrected as described above. (Please note: <br />The detail of correct and consistent operator name will be equally important when the bond documents are submitted.) <br />Exhibit E - Mining and Reclamation Maps (Rule 6.3.5) <br />There are several copies of the map sheet provided, but they are all essentially the same map. The reclamation map <br />depicts multiple structures as remaining onsite after reclamation. Given that the post-mining land use is being <br />amended to "general agriculture," please clarify that the reclamation map that was submitted was, in fact, intended to <br />be the reclamation map. (If you do verify that it is the reclamation map, please note that the features shown thereon <br />will be questioned as to their appropriateness during our technical adequacy review.) <br />Filing of Additional Materials with the County Clerk and :[recorder <br />Please ensure that the materials that you submit in response to this letter are also filed for public review at the county, <br />as the initial materials were, and a receipt obtained therefor. <br />Office of Office of <br />Mined Land Reclamation Denver Grand Junction • Durango Active and Inactive Mines
