(Page 2) <br />MINE ID # OR PROSPECTING ID #: M-1920-944 <br />INSPECTION DATE: 5/2!10 INSPECTORS INITIALS: RK <br />OBSERVATIONS <br />The Division conducted a construction monitoring inspection of the site on 5/8/10. The <br />main purpose of the inspection was to verify coverage during the weekend work <br />constructing the Phase 5 leach pad area. From now until the weather becomes a problem <br />possibly in early October, CC&V plans to work on weekends. The Division will as was the <br />case during past leach pad construction season, when clay and geomembrane liners are <br />being deployed. The Division had always increased the frequency of inspection during <br />these critical construction phases, under the Board ordered mandate. <br />Present during the inspection were Mr. Tim Comer, and Mr. Tom Shepard, of CC&V. <br />Clay liner deployment along the South Facing slopes of the pregnant pond for Phase 5 was <br />being conducted at the time of the inspection. The clay being deployed appeared to meet <br />the specifications; no rocks in excess of minus 11/2 were noted as approved in the clay <br />liner deployment specification. <br />Compaction of the clay along the Toe berm facing east was also taking place at the time <br />of the inspection. <br />Compaction of the sub grade slopes of the Phase 5 Leach pad was also in full force at the <br />time of the inspection. AMEC QA/QC personnel were on site verifying the QC of the <br />deployment area. <br />According to Mr. Sheppard, liner construction will continue through the weekend, mostly <br />Saturdays, unless a makeup day is need that will require the operator to work on Sunday, <br />through either Mid October, or end of October, depending on weather conditions, that has <br />limitations on liner deployment as approved in Amendment#9. <br />The small area of the off liner leak near the Phase 4C pad that was reported in March <br />inspection report had been deployed with clay liner in anticipation of geomebarne <br />deployment an anchor trench was being dug at the time of the inspection. <br />The deployed clay appeared to meet the required particle size distribution. The Division <br />allowed the operator to place the clay liner to utilize the favorable weather conditions, <br />prior to the submittal of the Technical Revision that is due at the end of June.
