(Page 2)
MINE ID # OR PROSPECTING ID #: M-1920-944
INSPECTION DATE: 5/2!10 INSPECTORS INITIALS: RK
OBSERVATIONS
The Division conducted a construction monitoring inspection of the site on 5/8/10. The
main purpose of the inspection was to verify coverage during the weekend work
constructing the Phase 5 leach pad area. From now until the weather becomes a problem
possibly in early October, CC&V plans to work on weekends. The Division will as was the
case during past leach pad construction season, when clay and geomembrane liners are
being deployed. The Division had always increased the frequency of inspection during
these critical construction phases, under the Board ordered mandate.
Present during the inspection were Mr. Tim Comer, and Mr. Tom Shepard, of CC&V.
Clay liner deployment along the South Facing slopes of the pregnant pond for Phase 5 was
being conducted at the time of the inspection. The clay being deployed appeared to meet
the specifications; no rocks in excess of minus 11/2 were noted as approved in the clay
liner deployment specification.
Compaction of the clay along the Toe berm facing east was also taking place at the time
of the inspection.
Compaction of the sub grade slopes of the Phase 5 Leach pad was also in full force at the
time of the inspection. AMEC QA/QC personnel were on site verifying the QC of the
deployment area.
According to Mr. Sheppard, liner construction will continue through the weekend, mostly
Saturdays, unless a makeup day is need that will require the operator to work on Sunday,
through either Mid October, or end of October, depending on weather conditions, that has
limitations on liner deployment as approved in Amendment#9.
The small area of the off liner leak near the Phase 4C pad that was reported in March
inspection report had been deployed with clay liner in anticipation of geomebarne
deployment an anchor trench was being dug at the time of the inspection.
The deployed clay appeared to meet the required particle size distribution. The Division
allowed the operator to place the clay liner to utilize the favorable weather conditions,
prior to the submittal of the Technical Revision that is due at the end of June. The
revision will detail the expanded liner deployment as part of the permanent drainage
problem remedial action that will be included for that area.
I & E Contact Address
NAME: MR.TIM COMER
OPERATOR: CC&V
STREET: P_0-ROX 191
CITY/STATE/ZIP: VICTOR C-0I ORADO- 80RA0
cc: r CE
F BL
F FS,
F HW
r HMWMD (CH)
r
r SE
F WQCD (CH)
F OTHER
