(Page 2) <br />MINE ID # OR PROSPECTING ID #: M-19ZL-01R- <br />INSPECTION DATE: 4/29/10 INSPECTOR=S INITIALS: MA(- <br />OBSERVATIONS <br />The inspection was conducted by Michael Cunningham of the Division of Reclamation, Mining and Safety <br />(Division). Lee Belmore of North Weld Gravel was also present for the inspection. The North Weld Gravel site <br />is located 4 miles northeast of Pierce, Colorado. The site is permitted for 80 acres; the maximum allowed <br />disturbed acreage is 20 acres. The post-mining land use is cropland. <br />The permit sign was posted at the entrance to the site and the boundary of the affected area was clearly <br />delineated per the requirements of Rule 3.1.12. The North Weld Gravel site is being mined in 4 phases; each <br />phase is approximately 20 acres in size. Currently mining is taking place in the southwest corner of Phase 1. <br />During the inspection, the disturbed area was traversed with a GeoXT GPS unit. The data collected in the field <br />were downloaded into GPS Pathfinder Office 3.10 software. The disturbed area was determined to be 10.37 <br />acres, or approximately half of Phase 1. Mining is currently progressing to the east end of Phase 1. The pit has <br />been mined to a depth of 12 feet. The topsoil has been salvaged and stockpiled along the west and south sides of <br />the disturbed area. The topsoil stockpiles are vegetated and did not show any signs of erosion. <br />Several product stockpiles along with screening equipment were located within the pit. Standing water was <br />observed at a low point in the pit, near the northern limit of disturbance. The water was likely from recent <br />precipitation events that preceded the inspection. No reclamation has been performed to date. The pit walls are <br />at an angle of approximately 1.75H:1 V. The Division currently holds a financial warranty in the amount <br />$10,000.00. The Division is required to hold a Financial Warranty in an amount sufficient to complete <br />reclamation of affected lands per C.R.S. 34-32.5-117(4)(b)(I). Based on the observations of the inspection, the <br />Division will recalculate the financial warranty for the North Weld Gravel site, Notice of the increased Financial <br />Warranty will be sent under separate cover. <br />During the inspection, the Operator indicated that production at the North Weld Gravel site may substantially <br />increase. The Operator was reminded that the maximum allowed disturbance at the site is not to exceed 20 acres <br />or a single Phase. If the Operator will disturb more than 20 acres, then a Technical Revision must be filed with <br />the Division.
