Section 10.2 Groundwater Monitoring Network: Exhibit 2 shows locations of existing and <br />proposed groundwater monitoring wells. The existing wells shall be shown on Exhibit 2 as <br />close to their actual locations as possible and with the appropriate well designations. Well <br />designations for existing wells shall be consistent with historic designations, and shall be <br />referenced consistently throughout the application. For example, existing monitoring well <br />MW-3AS shall continue to be referred to as MW-3AS and not MW-3 as is shown on the <br />Attachment 11 cover sheet. <br />11. Section 10:4.1 Sampling Methods: If bailers are used to purge monitoring wells, the wells <br />shall be purged of at least three wellbore storage volumes, or to dryness, whichever occurs first. <br />12. Section 10.6 Reporting: During the initial eight groundwater monitoring events, summary <br />reports shall be prepared and submitted to the Division for each event. The reports shall include <br />at a minimum: (1) groundwater elevation measurements, (2) well purging data, (3) field <br />parameter test data, (4) chain of custody documentation, (5) laboratory test data including <br />quality assurance and quality control information, and (6) a summary table that lists all detected <br />constituents and corresponding groundwater quality standards. Following the initial eight groundwater monitoring events, the summary reports shall also <br />include results of statistical analyses of groundwater data conducted pursuant to the <br />requirements set forth in Appendix B of the Solid Waste Regulations. <br />13. Section 12.1 Period: The Division requires the initially-approved post-closure care period to <br />be no less than 30 years. Pursuant to the provisions in Section 3.6.3 of the Solid Waste <br />Regulations, the post-closure period can either be increased or decreased by the Division after <br />consultation with the local governing jurisdiction. A request for such a modification to the <br />post-closure period typically would be made during the post-closure period, and would be <br />based on operating history, groundwater monitoring data, and other pertinent site-specific <br />information. <br />14. Attachment 15: Pursuant to the requirements of Section 1.8.1 of the Solid Waste Regulations, <br />closure and post-closure cost estimates shall be revised to reflect the cost of hiring a third party <br />to perform the closure and post-closure activities. The revised post-closure cost estimate shall <br />reflect 30 years of post-closure monitoring. <br />In addition to complying with the Division's Regulations, the applicant must comply with all relevant <br />federal, state, and local regulations including but not limited to the requirements of the Division of <br />Reclamation Mining and Safety, the Division of Water Resources, the Water Quality Control Division, <br />and the Air Pollution Control Division.
