(Page 3) <br />MINE ID # OR PROSPECTING ID #: M-1996-076 <br />INSPECTION DATE: 04/20/2010 <br />INSPECTOR'S INITIALS THM <br />and to the quality and quantity of water in surface and groundwater systems both during and after the mining <br />operation and during reclamation shall be minimized by complying with applicable Colorado water laws and <br />regulations governing injury to existing water rights. Lack of secondary containment is noted as a problem, the <br />corrective action(s) and correction date is specified on the last page of this report. <br />Various stock piles of material were observed during the inspection in both the "Middle" and "North" mining <br />locations. The operator stated that some of the boulders located in the "Middle Mine Area" have been imported <br />from a. residential property on Rapid Creek Road that belongs to the operator. A "Need for a State Reclamation <br />Permit" questionnaire is enclosed with this inspection report that the operator needs to fill out and return to this <br />office to determine if the activity at the residential property requires a reclamation permit. Also, pallets of imported <br />flagstone were observed in the "Middle Mine Area".
